摘　　要：目的探讨吉西他滨联合洛铂化疗对非小细胞肺癌患者血清癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)指标监测的临床意义。方法采用回顾性方法分析,选取127例非小细胞肺癌(均为肺鳞状细胞癌)患者的临床资料,根据治疗方法分为对照组(63例,给予吉西他滨联合顺铂治疗)与研究组(64例,给予吉西他滨联合洛铂治疗),比较2组患者临床疗效及对血清CEA、CYFRA21-1、NSE的影响及不良反应。结果研究组临床总有效率(67. 19%,43/64)明显高于对照组(39. 68%,25/63),差异有统计学意义(P <0. 05)。研究组的CEA、CYFRA21-1、NSE水平均低于对照组(P <0. 05)。研究组不良反应发生率(50. 00%)低于对照组(60. 32%),差异有统计学意义(P <0. 05)。结论非小细胞肺癌患者采用吉西他滨联合洛铂化疗治疗有显著疗效,可改善肿瘤标志物,降低不良反应,延长生存期,值得临床推广应用。Objective To explore the clinical significance of gemcitabine combined with luoplatin chemotherapy in monitoring serum carcinoembryonic antigen (CEA) ,neuron specific enolase ( NSE) , cytokeratin fragment antiogen 21 - 1 (CYFRA21 - 1) levels in patients with non - small cell lung cancer. Methods A retrospective analysis was conducted,the clinical data of 127 patients with non - small cell lung cancer (lung squamous cell carcinoma) were selected. They were divided into the control group (gemcitabine combined with cisplatin,n = 63) and the study group (gemcitabine combined with luoplatin,n = 64) . The clinical efficacy and the effects on serum CEA,CYFRA21 - 1 and NSE and adverse reactions were compared between the two groups. Results The total effective rate of the study group was 67. 19% (43 /64) ,which was significantly higher than that of the control group (39. 68%) (25 /63) , and the difference was statistically significant ( P<0.05) . The levels of CEA,CYFRA21 - 1 and NSE in the study group were lower than those in the control group (P<0. 05) . The incidence of adverse reactions in the study group was 50% lower than that in the control group (60. 32%) , and the difference was statistically significant ( P < 0.05) . Conclusion Non - small cell lung cancer patients with gemcitabine combined with luoplatin chemotherapy has significant effect. It can improve tumor markers,reduce adverse reactions and prolong life. It is worthy of clinical application.
