调神疏肝法针刺结合SSRI类药物治疗抑郁症临床研究

摘　　要：目的:观察调神疏肝法针刺结合选择性5-羟色胺再摄取抑制剂(SSRI)类药物治疗抑郁症的临床疗效和安全性。方法:纳入82例抑郁症肝气郁结证或忧郁伤神证患者,随机分为治疗组和对照组各41例。2组均使用SSRI类抗抑郁药治疗,治疗组加用调神疏肝法针刺,2组均治疗6周。分别于治疗前及治疗后1、2、4、6周采用24项汉密尔顿抑郁量表(HAMD)和Asberg氏抗抑郁剂副反应量表(SERS)评定患者的抑郁症状和用药安全性。结果:治疗6周后,治疗组疗效优于对照组,差异有统计学意义(P <0.05)。治疗1周,治疗组HAMD评分较治疗前降低,差异有统计学意义(P <0.01),对照组治疗前后评分无统计学差异(P>0.05);治疗2、4、6周,2组HAMD评分均低于治疗前,差异均有统计学意义(P <0.05);治疗1周、6周,治疗组HAMD评分均低于对照组,差异均有统计学意义(P <0.05)。治疗1周后,治疗组的SERS评分均低于对照组,差异均有统计学意义(P <0.01)。结论:应用调神疏肝法针刺结合SSRI类药物治疗抑郁症肝气郁结证或忧郁伤神证能够提高临床治愈率,改善抑郁症状,并且减少了药物的副反应,是一种安全有效的治疗手段。Objective:To observe the clinical effect and safety of acupuncture in method of regulating mentality and smoothing liver combined with selective serotonin reuptake inhibitors(SSRI)medications for depression.Methods:Selected 82 cases of patients with depression together with liver qi stagnation syndrome or depression debilitating mentality syndrome,and divided them into the treatment group and the control group randomly,41 cases in each group.Both groups received SSRI medications,while the treatment group additionally received acupuncture in method of regulating mentality and smoothing liver.The treatment for both groups lasted for 6 weeks.Before treatment and in the 1st,2nd,4th and 6th week after treatment,evaluated the status of depression in patients and safety via24-item Hamilton depression scale(HAMD)and Asberg side-effect rating scale(SERS).Results:After treatment for 6 weeks,the clinical effect in the treatment group was better than that in the control group,difference being significant(P<0.05).In the 1st week of treatment,scores of HAMD in the treatment group were lower than those before treatment,difference being significant(P<0.01),and there was no significant difference being found in the control group before and after treatment(P>0.05).In the 2nd,4th and 6th week of treatment,scores of HAMD in both groups were lower than those before treatment,differences being significant(P<0.05).In the 1st and 6thweek of treatment,scores of HAMD in the treatment group were lower than those in the control group,differences being significant(P<0.05).After treatment for 1 week,scores of SERS in the treatment group were all lower than those in the control group,differences being significant(P<0.01).Conclusion:The application of acupuncture in method of regulating mentality and smoothing liver combined with SSRI medications for depression with liver qi stagnation syndrome or depression debilitating mentality syndrome can improve the clinical cure rate,improve the symptoms of depression and reduce the side effects of medic
