硬膜外阻滞复合全麻在老年腹部手术中的效果

摘　　要：目的探究硬膜外阻滞复合全身麻醉中老年腹部手术患者凝血纤溶功能、心肌损害指标的变化情况。方法将2017年5月-2018年5月江门市新会区人民医院收治的老年腹部手术患者80例,随机分为对照组和观察组各40例。对照组仅采用静脉插管全麻,观察组开展硬膜外阻滞复合全身麻醉,比较两组凝血纤溶功能和心肌损害指标。结果与对照组比较,观察组患者的凝血纤溶功能和心肌损害指标改善明显,差异有统计学意义(P<0.05)。结论在腹部手术老年患者中开展硬膜外阻滞复合全身麻醉,可改善凝血纤溶功能、心肌损害指标,值得临床推广应用。Objective To investigate the changes of coagulation and fibrinolysis and myocardial damage in elderly patients undergoing epidural anesthesia combined with general anesthesia.Methods 80 elderly patients undergoing abdominal surgery admitted to Xinhui District People's Hospital,Jiangmen City from May 2017 to May 2018 were randomly divided into control group and observation group with 40 cases in each group.The control group was given total anesthesia with venous intubation,the observation group was given epidural block combined with general anesthesia.The coagulation and fibrinolysis function and myocardial damage index of the two groups were compared.Results Compared with the control group,the coagulation and fibrinolysis function and myocardial damage index of the observation group improved significantly,the differences were statistically,significant (P<0.05).Conclusion Epidural block combined with general anesthesia in elderly patients undergoing abdominal surgery can improve the coagulation and fibrinolysis function and myocardial damage index,which is worthy of clinical investigation.
