适形调强放疗联合替吉奥和奈达铂治疗局部晚期食管癌的疗效观察

摘　　要：目的探讨适形调强放疗联合替吉奥和奈达铂治疗局部晚期食管癌的临床效果。方法选取2011年7月至2016年6月间江苏大学附属医院收治的62例局部晚期食管癌患者,根据治疗方法不同进行分组。采用适形调强放疗的31例患者纳入对照组,在适形调强放疗治疗基础上采用替吉奥联合奈达铂化疗的31例患者纳入观察组,比较两组患者的治疗效果、生活质量和无疾病进展生存时间。结果观察组患者治疗有效率为64. 5%,疾病控制率为100. 0%,优于对照组的38. 7%和80. 6%,两组比较,差异均有统计学意义(均P <0. 05)。观察组患者治疗后生活质量变化程度优于对照组患者,差异有统计学意义(P <0. 05)。观察组患者无进展生存时间中位数为15. 0个月,95%CI为6. 290～23. 710个月,对照组分别为11. 0个月和8. 822～13. 178个月,两组比较,差异有统计学意义(P <0. 05)。结论适形调强放疗联合替吉奥和奈达铂治疗局部晚期食管癌患者,可提升治疗效果,延长疾病进展时间,值得临床推广。Objective To investigate the clinical efficacy of intensity-modulated radiotherapy com- bined with tegafur and nedaplatin for locally advanced esophageal cancer.Methods A total of 62 patients with locally advanced esophageal cancer treated at Jiangsu University Affiliated Hospital from Junly 2011 to June 2016 were selected.According to the therapy patients received,they were divided into anobservation group and a control group with 31patients in each group.The observation group received intensity-modula- ted radiotherapy combined with tegafur and nedaplatin and the contro group only received intensity-modulated radiotherapy.The treatment efficacy,quality of life and progression-free survival were compared between the two groups.Results The treatment efficacy was 64.5%and disease control rate was 100.0% for the observation group which was better than 38.7%and 80.6%,respectively of the control group (all P< 0.05).The changes in quality of life in the observation group was better than that in the control group (P< 0.05).The median progression-free survival time was 15.0(95%CI 6.290-23.710)months for the observation group and 11.0(8.822-13.178)months for the control group (P<0.05).Conclusion Intensity-modulated radiotherapy combined with tegafur and nedaplatin in the treatment of locally advanced esophageal cancer can improve the therapeutic efficacy and prnlong disease progression,and it is worth promoting in clinical work.
