摘　　要：目的:观察CT引导下三维后装联合三维适形放疗治疗宫颈残端癌的疗效及安全性。方法:回顾性分析我院2009-01～2014-01行CT引导下三维后装联合三维适形放疗的42例宫颈残端癌患者的临床资料,观察患者疗效及毒性反应,采用Kaplan-Meier法计算生存率。结果:本组患者盆腔外照射剂量为46～68Gy,后装剂量为5～25Gy。放疗结束后疗效评估:完全缓解16例(38.10%)、部分缓解17例(40.48%)、稳定9例(21.42%),放疗有效率为78.57%。毒性反应:放射性膀胱炎2例,放射性肠炎1例,血液系统毒性34例。随访至2018-04-01,5年无进展生存率及总生存率分别为74.60%和73.20%。结论:CT引导下三维后装联合三维适形放疗是治疗宫颈残端癌的一种安全有效的方法。Objective:To analyze the efficacy and safety of CT-guided three-dimensional brachytherapy combined with three-dimensional conformal radiotherapy in patients with cervical stump carcinoma.Methods:The clinical data of the patients with cervical stump carcinoma treated with CT-guided three-dimensional brachytherapy combined with threedimensional conformal radiotherapy in our hospital between January 2009 to January 2014 were analyzed.The efficacy and complications of the patients were evaluated.Survival rate were calculated by the Kaplan-Meier.Results:The cervical stump carcinoma patients with pelvic external exposure dose was 46-68 Gy,after loading dose was 5-25 Gy.16 cases(38.10%)achieved CR(complete remission),17 cases(40.48%)achieved PR(partial remission),9 cases(21.42%)achieved SD(stable disease),the total effective rate was 78.57%.Among the42 cases of the patient,toxicity of the blood system in 34 cases,radiocystitis occurred in2 cases,and radiation proctitis was happened in 1 case.Followed-up to April 2018,the progression free survival rate was 74.60%,the overall survival rate was 73.20%.Conclusion:Three-dimensional brachytherapy combined with external-beam radiotherapy for carcinoma of cervical stump is feasible and safe.
