摘 要:目的为临床不同方向下进行前方牵引时,不同形态微钛板的选用提供参考。方法构建6种改良型微钛板及上颌骨的三维有限元模型。对该模型牵引位点施加大小为4.9 N/侧,方向为与上颌牙合平面夹角为0°~50°向前下的力,分析各工况下微钛板所受应力及其固位螺钉所受拉力和应力。结果 (1)不同微钛板均可固定于上颌骨颧牙槽嵴处且无受力过大或应力中断现象。(2)固位螺钉受力具有差异性。角度为0°时,Y3型微钛板固位螺钉受力离散程度较小为3.899 N,10°~40°时,L3型微钛板固位螺钉受力离散程度较小分别为4.544、4.170、3.820、3.547 N,50°时,L2型微钛板固位螺钉受力离散程度较小为2.687 N。结论微钛板辅助上颌骨发育不足患者在不同方向下进行前方牵引时,应考虑选用不同形态:牵引方向与牙合平面夹角为0°时,选用Y3型微钛板;夹角为10°~40°时,选用L3型微钛板;夹角为50°时,选用L2型微钛板。Objective To provide a reference for the selection of modified miniplate implants in different clinical directions for maxillary protraction. Methods The three-dimensional CAD model of six forms of modified miniplate implants was constructed and assembled with maxillary model to obtain the final model. A force of 4.9 N on each side was applied to the traction sites of the model,and the direction was 0°to 50°with the maxillary occlusal plane. Then the stress on the modified miniplate implants and the tension and stress on the retaining screw were analyzed under different working conditions. Results Different modified miniplate implants could be fixed to the mandibular zygomatic alveolar ridge without too much tension or broken stress. The force of retention screw was different,and it showed different trends with the change of angles. When the angle was 0°,Y3 miniplate implants retention screw was 3.899 N,and when the angle was 10°-40°,L3 miniplate implants retention screw was 4.544,4.170,3.820,3.547 N and when the angle was 50°,L2 miniplate implants retention screw was 2.687 N. Conclusion In order to achieve more stable curative effect,the modified miniplate implants should be used to assist the maxillary hypoplasia patients to draw in different directions. When the angle between the traction direction and occlusal plane is 0 degree,Y3 miniplate implants should be used. When the angle between the traction direction and the occlusal plane is 10-40 degrees,L3 miniplate implants is selected. L2 miniplate is selected when the angle between traction direction and occlusal plane is 50 degrees.
