摘 要:目的本研究通过利用锥形束CT(cone beam computed tomography,CBCT)进行扫描重建有上前牙前突患者的上下颌骨,进行分析对比微种植体植入后牙区颌骨不同部位骨密度的差异,为临床上能更准确地定位微种植体支抗植入位置方面提供参考。方法将20例有上前牙前突的患者利用锥形束CT对其口腔颌面部进行扫描及三维重建,在上颌后牙区第二前磨牙与第一磨牙区、第一磨牙与第二磨牙距离牙槽嵴顶3、5、7、9 mm处,先以近远中向选取上后牙区各牙根间牙槽嵴中心层,再在唇腭向选取矢状面上后牙区距离最外层骨皮质2、4、6 mm处进行骨密度的测量,比较不同区域内骨密度的差异。结果在上颌骨后牙段不同高度同一深度,不同深度同一高度,骨密度值之间的对比均存在统计学差异(P<0.001)。在距离骨皮质相同深度时,距牙槽嵴顶高度为7 mm处骨密度值大于其他各组,而在同一高度时,随着深度的增加骨密度值减少。结论通过CBCT测量分析不同高度与深度植入上颌后牙区的外周骨密度大小,在距离牙槽嵴顶高度为7 mm处骨密度值大于其他位置。可以为微种植体植入提供参考。Objective Based on the process of scanning the mandible and maxilla of patients with upper anterior teeth protrusion by cone beam CT,to analyze and compare the changes of micro implant bone density in different positions of implantation of the posterior region of jaw,and to provide clinical reference for the presice location of micro implant anchorage implants.Methods The oral and maxillofacial regions of 20 cases of patients with upper anterior teeth protrusion were scanned by cone beam CT and three-dimensional reconstruction was conducted.In the first molar and the second molar second posterior maxillary premolar and the first molar area,distance 3,alveolar crest 5,7 and 9mm,the near far posterior root area between the ridge center layer was first selected,and then coro- nal posterior region of the outer cortical distance of 2,4 and 6mm was selected for the measurement of bone mineral density in lip and palate.The difference of bone mineral density in different areas was compared.Results There were statistical differences in bone mineral density at the same depth with different heights,at the same height with different depths(P<0.001).At the same depth as the bone cortex,the bone density was higher than that of the other groups at the height of 7mm from the crest of the alveolar ridge.At the same height,the bone density decreased with the increase of the depth.Conclusion The peripheral bone density of the maxillary posterior region is measured by CBCT at different heights and depths,and the bone density is higher than the other locations at the 7mm from the crest of alveolar ridge,which can provide reference for micro implant.
