超声乳化技术联合人工晶体植入术治疗青光眼术后白内障的临床效果

摘　　要：目的探讨超声乳化技术联合人工晶体植入术治疗青光眼术后白内障的临床效果。方法选取我院2014年6月～2017年6月收治的80例(90眼)青光眼术后白内障患者作为研究对象,所有病例应用超声乳化技术联合人工晶体植入术治疗,观察患者的治疗效果。结果患者术前的视力均<0.1,术后均有所改善,视力均>0.1,其中0.1≤视力≤0.3者49例、>0.3患者31例。患者均手术矫正成功,术中患者未出现晶状体后囊膜破裂、滤过泡损伤,少数病例出现+++以上房水闪辉、角膜水肿、瞳孔轻度散大并发症。患者术后的角膜内皮细胞计数低于术前,差异有统计学意义(P<0.05)。结论超声乳化技术联合人工晶体植入术治疗青光眼术后白内障的安全性高,术后可明显改善患者的视力,角膜内皮细胞损伤也较少,但可能存在角膜水肿、房水闪辉等并发症,需积极进行激素、对症、散瞳等治疗措施,以促进患眼康复。Objective To explore the clinical effect of phacoemulsification combined with intraocular lens implantation treating cataract after glaucoma surgery. Methods All of 80 cases(90 eyes) with cataract after glaucoma surgery treated in our hospital from June 2014 to June 2017 were selected as the study subjects. All cases were treated with phacoemulsification combined with intraocular lens implantation. The treatment effect of the patients was observed. Results The vision of patients before operation was <0.1, and all of them improved after operation, all >0.1. Among them, there were 49 cases with 0.1 ≤vision ≤0.3, 31 cases with vision >0.3. The patients were successfully surgically corrected.There was no rupture of posterior capsule of the lens and filtering bleb injury during operation. In a few cases, there were complications such as aqueous flare of more than +++, corneal edema and mild dilatated pupil. The number of corneal endothelial cells after operation were lower than those before operation(P <0.05). Conclusion The safety of phacoemulsification combined with intraocular lens implantation treating cataract after glaucoma surgery is high and can improve the vision of patients significantly after operation, and the corneal endothelial cell damage is fewer, but there may be complications such as corneal edema, aqueous flash and so on. It is necessary to actively carry out hormone therapy, symptomatic treatment and other treatment measures to promote the recovery of the affected eyes.
