低分子肝素钙联合逐级加压弹力袜预防妇科恶性肿瘤患者术后下肢深静脉血栓的临床研究

摘　　要：目的探讨应用低分子肝素钙联合逐级加压弹力袜预防妇科恶性肿瘤患者术后下肢深静脉血栓的效果。方法选择2017年6月至2018年6月平顶山市第一人民医院80例妇科恶性肿瘤手术患者并随机分成观察组和对照组,每组40例。对比术后5d两组血清学指标、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、下肢周径、血流速度、并发症发生率。结果观察组术后5d纤维蛋白原、D-二聚体水平均低于对照组,差异均有统计学意义(P<0.01);APTT、PT均高于对照组,差异均有统计学意义(P均<0.01)。观察组术后5d下肢周径及血流速度分别为(21.84±1.82)cm和(0.98±0.24)m/s,优于对照组[(25.73±1.67)cm、(0.61±0.05)m/s],差异均有统计学意义(P均<0.01,)。观察组下肢深静脉血栓发生率为5.0%(2/40),与对照组[22.5%(9/40)]比较,差异有统计学意义(χ^25.38,P<0.05)。观察组并发症发生率为5.0%(2/40),与对照组[27.5%(11/40)]比较,差异有统计学意义(χ^27.54,P<0.05)。结论低分子肝素钙联合逐级加压弹力袜应用于妇科恶性肿瘤术后可以有效改善患者血流状态,预防深静脉血栓形成。ObjectiveTo investigate the effect of low molecular weight heparin calcium combined with graduated compression stockings on preventing deep venous thrombosis of lower extremity in postoperative patients with gynecological malignant tumors.MethodsEighty patients with gynecological malignant tumors in the First People's Hospital of Pingdingshan from June 2017 to June 2018 were randomly divided into observation group and control group, with 40 cases in each group. The serological indexes, APTT, PT, peripheral diameter of lower extremity, blood flow rate and the incidence of complications were compared between the two groups 5 days after surgery.ResultsAt the 5th day after the operation, the levels of fibrinogen and D-dimer in the observation group were lower than those of the the control group, and the differences between the two groups were all significant (P<0.01); the levels of APTT and PT were higher than those of the control group, and the differences between the two groups were all significant (P<0.01). At the 5th day after the operation, the lower limb circumference and blood flow velocity in the observation group were (21.84±1.82) cm and (0.98±0.24) m/s, which were better than those of the control group [(25.73±1.67)cm、(0.61±0.05)m/s], and the differences were all significant (P all<0.01). The incidence of deep venous thrombosis of lower extremity was 5.0% (2/40) in the observation group and 22.5% (9/40) in the control group, there was significant difference (χ^25.38, P<0.05). The incidence of complication was 5.0% (2/40) in the observation group and 27.5% (11/40) in the control group, there was significant difference(χ^27.54, P<0.05).ConclusionsLow molecular weight heparin calcium combined with graduated compression stockings can effectively improve the blood flow and prevent deep venous thrombosis in postoperative patients with gynecological malignant tumors.
