摘　　要：目的总结嗅神经母细胞瘤临床资料,探讨临床特点及合理治疗方法。方法回顾性分析1999年8月～2016年6月间在北京大学第一医院收治的12例嗅神经母细胞瘤患者的临床资料并结合文献分析其临床特征、治疗方式及其预后。男女各6例,年龄30～62岁,中位年龄53.0岁。Kadish分期A、B期各1例,C期10例。治疗方式:手术及放化疗5例,单纯手术治疗7例。结果随访1～12年,中位随访时间3.5年,1年生存率75.0%(9/12),3年生存率63.6%(7/11)。随访期间死亡6例,Kadish分期均为C期。结论成人嗅神经母细胞瘤多以头痛、鼻堵及涕中带血为首发症状;Kadish临床分期与预后有关。颈淋巴结转移是最常见的转移途径,对于嗅神经母细胞瘤应重视长期随访。OBJECTIVE To summarize the clinical data of patients with olfactory neuroblastoma(ONB),and explore the clinical characteristics and rational treatment of olfactory neuroblastoma.METHODS The clinical data of 12 patients with olfactory neuroblastoma in Peking University First Hospital from August 1999 to June 2016 were retrospectively analyzed.Of the patients,6 cases were males and 6 cases were females,and their median age was 53.0 years.The patients were staged according to the Kadish staging system,1 case was in A,1 case was in B,10 cases were in C.Two patients had cervical lymph nodes metastasis.Seven patients had surgery alone, whereas the others received surgery and chemoradiotherapy postoperatively.RESULTS Duration of follow-up ranged from 1 to 12 years(median,3.5 years).The 1-and 3-year survival rates were 75.0%(9/12)and 63.6%(7/11), respectively.Six patients with Kadish C stage were died of disease.CONCLUSION Headache,nasal obstruction and bloody rhinorrhea were the most common clinical manifestations for adult olfactory neuroblastoma.The Kadish staging system can effectively predict the prognosis of the patients.The region of the main metastasis was cervical lymph nodes.Long-term follow-up of ONB should be mandatory.
