社区双相情感障碍患者躯体疾病的共病率及相关因素分析

摘　　要：目的调查社区双相情感障碍(Bipolar Disorder,BD)患者躯体疾病的共病率及相关因素。方法采用多阶段分层系统整群抽样方法抽取≥15周岁的湖南省123个县区的常住居民,以增补后的一般健康问卷(GHQ-12)及重性精神疾病异常行为线索作为筛选工具、《DSM-Ⅳ-TR轴Ⅰ障碍定式临床检查》(SCID-I/P)作为诊断工具。在调查的72999居民中,诊断为BD的75例患者作为研究对象,对其躯体疾病的共病率及相关因素进行调查分析。结果共病躯体疾病者46例(61.33%),共病的躯体疾病以慢性病为主,依次为心血管系统疾病、消化系统疾病、内分泌系统疾病等。患者年龄为共病躯体疾病相关因素,P<0.05。结论 BD患者和躯体疾病的共病率高,与年龄等有密切关联。Objective To investigate the comorbidity rate and related factors of somatic diseases in Bipolar Disorder Patients in Community. Methods Using multistage stratified random cluster sampling method,123 county residents of Hunan province(≥ 15 years old)were selected. They were administered with supplementary general health questionnaire(GHQ-12)and abnormal behavior clue of heavy mental disorder. The DSM-Ⅳ-TR Axis I DisordersPatient Edition(SCID-I/P)was used as a diagnostic tool. Total 72999 residents were surveyed,75 patients who diagnosed with bipolar disorder were included in the study. The co-morbidity rate and related factors of somatic diseases were investigated and analyzed. Results 46 cases(61.33%)had physical diseases among the 75 patients,whose mainly were chronic diseases,followed by cardiovascular diseases,digestive diseases,endocrine diseases and so on. The patient's age was the related factors of comorbidity(P<0.05). Conclusion The comorbidity rate of somatic diseases in Bipolar Disorder Patients in Community was high,which was closely related with age.
