齐拉西酮或奥氮平联用丙戊酸钠对双相Ⅰ型患者认知功能影响的对照研究

摘　　要：目的比较齐拉西酮或奥氮平联用丙戊酸钠对双相Ⅰ型患者认知功能的影响。方法将我院2015年2月～2016年1月收治的120例双相Ⅰ型患者随机分为A组(齐拉西酮联合丙戊酸钠)和B组(奥氮平联合丙戊酸钠)各60例,并给予相应的药物治疗2月,对两组认知功能影响效果进行比较。结果两组患者经治疗后,贝克躁狂量表(BRMS)评分均有明显下降,但两组患者之间没有差异(P>0.05);蒙特利尔认知功能评定量表(MoCA)评分在交替连线测试、数字符号、数字广度、卡片分类测试方面比较,差异均具有统计学意义(P<0.05);个人和社会功能量表(PSP)评分均有明显提高,两组之间存在差异(P<0.05)。结论在双相Ⅰ型患者治疗中丙戊酸钠联合齐拉西酮或奥氮平均可取得理想的认知功能改善效果,但丙戊酸钠联合齐拉西酮效果更佳。Objective To compare the effects of ziprasidone and olanzapine in combination with sodium valproate on cognitive function in bipolar I patients.Methods 120patients with bipolar type I were randomly divided into group A (ziprasidone combined with sodium valproate )and group B (olanzapine combined with sodium valproate ) from February 2015to January 2016.And the corresponding medication was given to every group for two month and the cognitive effects of the two groups were compared.Results After treatment,the scores of Bech-Rafaelsen Mania Rating Scale (BRMS)decreased obviously in both groups,but the differences were not statistically significant (P >0.05). The scores of Montreal Cognitive Assessment (MoCA)were significantly different in alternating connection test,num- ber sign,digital breadth and card classification test (P <0.05).Personal and Social Performance scale (PSP)scores were significantly increased,and the differences were statistically significant (P <0.05).Conclusions In the biphasic type I patients,valproate combined with ziprasidone or olanzapine achieved optimal cognitive performance improvement,but sodium valproate combined with ziprasidone performed better.
