丙戊酸钠联合喹硫平治疗双相情感障碍躁狂发作的应用效果探究

摘　　要：目的探讨丙戊酸钠联合喹硫平治疗双相情感障碍躁狂发作的应用效果。方法 90例双相情感障碍躁狂发作患者,根据随机数字表法分为对照组和观察组,每组45例。对照组给予丙戊酸钠治疗,观察组则给予丙戊酸钠联合喹硫平治疗。比较两组患者的临床治疗效果、症状缓解时间、治疗前后汉密尔顿抑郁量表评分及贝克-拉范森躁狂量表(BRMS)评分、药物不良反应发生情况。结果观察组患者的临床治疗总有效率97.78%(44/45)显著高于对照组的68.89%(31/45),差异具有统计学意义(P<0.05)。治疗前,两组患者的汉密尔顿抑郁量表评分、BRMS评分比较,差异无统计学意义(P>0.05);治疗后,观察组患者的汉密尔顿抑郁量表评分、BRMS评分均低于对照组,差异具有统计学意义(P<0.05)。观察组患者的症状缓解时间(1.11±0.41)周短于对照组的(3.42±1.25)周,差异具有统计学意义(P<0.05)。观察组患者的药物不良反应发生率6.67%与对照组的4.44%比较,差异无统计学意义(P>0.05)。结论丙戊酸钠联合喹硫平治疗双相情感障碍躁狂发作的应用效果确切,可有效改善症状,控制躁狂发作,减轻抑郁,且无严重不良反应,安全有效,值得推广应用。Objective To discuss the application effect of sodium valproate combined with quetiapine in the treatment of bipolar disorder manic episode. Methods A total of 90 patients with bipolar disorder manic episode were divided by random number table into control group and observation group, with 45 cases in each group. The control group was treated with sodium valproate, and the observation group was treated with sodium valproate combined with quetiapine. Comparison were made on clinical treatment effect, symptom remission time, Hamilton depression scale score, Baker-Raphson Mania Scale(BRMS) score before and after treatment and occurrence of adverse drug reactions between the two groups. Results The observation group had significantly higher total effective rate of clinical treatment as 97.78%(44/45) than 68.89%(31/45) in the control group, and the difference was statistically significant(P<0.05). Before treatment, both groups had no statistically significant difference in Hamilton depression scale score and BRMS score(P >0.05). After treatment, the observation group had lower Hamilton depression scale score and BRMS score than the control group, and the difference was statistically significant(P<0.05). The observation group had shorter symptom remission time as(1.11±0.41) weeks than(3.42±1.25) weeks in the control group, and the difference was statistically significant(P<0.05). The observation group had no statistically significant difference in incidence of adverse drug reactions as 6.67%, comparing with 4.44% in the control group(P >0.05). Conclusion Combination of sodium valproate and quetiapine shows affirmative effect in treating bipolar disorder manic episode, and can effectively improve symptoms, control manic episodes and alleviate depression with no serious adverse reactions. It is safe and effective and worth popularizing and applying.
