支气管袖状肺叶切除术治疗中心型肺癌的效果分析

摘　　要：目的比较支气管袖状肺叶切除术和全肺切除术治疗中心型肺癌的临床效果。方法对2008—2016年辽河油田总医院收治的131例中心型肺癌患者的临床资料进行回顾性分析,根据患者所采用手术术式的不同分为对照组(全肺切除术)61例和观察组(支气管袖状肺叶切除术)70例,比较两组患者的手术情况、术后并发症发生率及术后1年复发率。结果两组手术时间、术中出血量、胸腔引流量、拔引流管时间、术后卧床时间及并发症发生率差异均有统计学意义(P<0.05),术后1年复发率差异无统计学意义(P>0.05)。结论中心型肺癌行支气管袖状肺叶切除术治疗,具有术后恢复快、并发症发生率低优势,而复发情况与全肺切除术相当,建议临床推广使用。Objective To compare the clinical effects of bronchial sleeve lobectomy and pneumonectomy in the treatment of central lung cancer.Methods Clinical data of 131 patients with central lung cancer admitted to Liaohe Oilfield General Hospital from 2008 to 2016 were retrospectively analyzed.The patients were divided into control group (61 cases of pneumonectomy) and observation group (70 cases of bronchial sleeve lobectomy) according to the different surgical treatment methods.The surgical conditions,incidence of complications and one year postoperative recurrence rate were compared between the two groups.Results There were significant differences in operation time,intraoperative bleeding volume,thoracic drainage volume,drainage tube extraction time,bedridden time and incidence of complications between the two groups(P < 0.05).There was no significant difference in one year postoperative recurrence rate between the two groups(P > 0.05).Conclusion Bronchial sleeve lobectomy has the advantages of quick recovery and low incidence of complications for central lung cancer.The recurrence rate of bronchial sleeve lobectomy is similar to that of pneumonectomy.It is recommended for clinical application.
