摘  要:目的对比TF与M3机用镍钛器械用于下颌第二磨牙C形根管治疗的临床疗效。方法选取需要做根管治疗的40例下颌第二磨牙C形根管患者(40颗患牙)作为研究对象,随机分为观察组(n=20)和对照组(n=20)。其中对照组采用M3机用镍钛系统预备根管,观察组采用TF机用镍钛系统预备根管,2组均采用热牙胶完成根管充填。分别对2组的根管预备时间、根管预备并发症、疼痛反应、根管充填效果进行评价。结果观察组的根管平均预备时间明显短于对照组(P<0.05)。2组发生的并发症包括根尖偏移、台阶形成、根管壁穿孔,其中观察组的并发症发生率稍低于对照组,但差异无统计学意义(P>0.05)。观察组术后Ⅰ~Ⅳ级疼痛发生率与对照组差异无统计学意义(P>0.05)。2组均无器械分离,预备效果佳,根管充填有效率均达100%。结论TF与M3机用镍钛系统在预备下颌第二磨牙时均能较好地维持根管的原解剖形态,在一定程度上减少了根管预备后疼痛程度及并发症,并提高了根管治疗效果。Objective To compare the clinical effect of TF and·M3 apparatus used in the C -shaped root canal of mandibular second molars.Methods .A total of 40 patients (40 teeth)with C -shaped root canals in mandibular second molars undergoing root canal therapy were randomly divided into an observation group (n =20) and a control group (n = 20).In the control group,M3 was used to prepare the root canal with nickel -titanium system.In the observation group, TF was used to prepare the root canal with nickel-titanium system.Both groups were filled with hot tooth glue to complete root.canal filling.The root canal preparation time,complications,pain response and filling effect of the two groups were evaluated.Results The average root.canal preparation time in the observation group was significantly shorter than that in the control group (P <0.05).Complications in the two groups included deviation of apical root of tooth,step formarion and perforation of the root canal wall.The incidence of complications in the observation group was slightly lower than that in the control group,but the difference was.not statistically significant (P >0.05).After operation,there was no significant difference in the incidence of I -IV pain between the two groups (P >0.05).Both groups had no separation of instruments.The preparatory effect was good and the effective rate of root canal filling was 100% in both groups. Conclusions Both TF and M3 nickel -titanium systems can maintain the original anatomy of the root canal in the preparation of mandibular second molars,reduce the degree of pain and complications after root canal preparation to a certain extent,and impro.ve the efficacy of root canal treatment.
