摘　　要：目的探讨伴恶性肿瘤史患者胸膜孤立性病变(PSL)的诊断和鉴别诊断。方法搜集87例伴恶性肿瘤史患者PSL的资料,通过对既往恶性肿瘤病理类型、肿瘤确诊时间间隔及CT影像特征等因素进行分析,探讨此类病灶定性诊断的可能。采用χ^2或Fisher确切概率法,以及趋势性卡方检验进行统计学分析。结果 87例PSL中转移瘤组66例,非转移瘤组21例。两组PSL的性质与既往恶性肿瘤病理类型的相关性,差异有统计学意义,Fisher值=20.95,P<0.05。两组PSL与既往恶性肿瘤确诊的时间间隔有线性趋势,P=0.001。两组PSL的性质与CT各项影像学特征,均无统计学差异,P >0.05。结论伴恶性肿瘤史患者PSL的性质与既往恶性肿瘤的病理类型有相关性,两组PSL确诊的时间间隔有线性趋势,但PSL的性质与影像学特征无明显相关性。Objective To investigate the diagnosis and differential diagnosis of pleural solitary lesions(PSL) in patients with history of malignant tumors.Methods The data of PSL in 87 patients with history of malignant tumors were collected.The pathological types of previous malignant tumors,the time interval of diagnosis of tumors and the characteristics of CT images were analyzed to explore the possibility of qualitative diagnosis of such lesions.Statistical analysis was performed using the χ^2 or Fisher exact probability method and the trending chi-square test.Results Of the 87 patients with PSL,66 were in the metastatic tumor group and 21 in the non-metastasis group.The relationship between the nature of PSL in the two groups and the pathological type of previous malignant tumors was statistically significant.Fisher value =20.95,P < 0.05.There was a linear trend in the time interval between the two groups of PSL and previous malignant tumors,P = 0.001.There was no significant difference in the nature of PSL between the two groups and CT imaging features,P > 0.05.Conclusion The nature of PSL in patients with history of malignant tumors is correlated with the pathological type of previous malignant tumors.There is a linear trend in the time interval between the two tumors,but there is no significant correlation between the nature of PSL and imaging features.
