睾丸肿瘤及肿瘤样病变CT与MR表现

摘　　要：目的探讨睾丸肿瘤及肿瘤样病变影像表现。方法回顾性分析经手术病理证实睾丸肿瘤与肿瘤样病变38例,27例行MRI平扫及增强扫描,15例行CT平扫及增强扫描。结果生殖细胞瘤占52.6%(20/38),其中精原细胞瘤占26.3%(10/38),肿瘤均有分隔,增强扫描分隔明显强化;非精原细胞瘤占26.3%(10/38),混合型生殖细胞瘤4例,胚胎性癌3例,畸胎瘤2例,密度或信号不均,表皮样囊肿1例,信号均匀,无强化;非生殖细胞瘤占21.0%(8/38),肉瘤3例,转移瘤1例,腺样腺瘤2例,海绵状血管瘤1例,淋巴瘤1例,肿瘤有各自特点;肿瘤样病变占26.3%(10/38),结核4例,脓肿1例,慢性炎症3例,肉芽肿1例,临床表现明显,均累及附睾及皮肤,血肿1例,有外伤史。结论睾丸肿瘤及肿瘤样病变种类繁多,影像学作为一种辅助检查具有重要的价值,其CT/MR表现具有一定特征性,临床症状、年龄、相关实验室检查等可帮助诊断及鉴别诊断。Objective To investigate the imaging findings of testicular tumors and tumor-like lesions.Methods A retrospective analysis of 38 cases of testicular tumors and tumor-like lesions confirmed by operation and pathology,27 cases of MRI plain scan and enhanced scan,15 cases of CT plain scan and enhanced scan.Results The germ cell tumors accounted for 52.6%(20/38),of which 26.3%(10/38) were seminoma,the tumors were septated,and the septae displayed obvious enhancement;the non-seminoma accounted for 26.3%(10/38),4 cases of mixed germ cell tumor,3 cases of embryonal carcinoma,2 cases of teratoma,density or signal unevenness,1 case of epidermoid cyst,uniform signal,no enhancement;non-germ cell tumor accounted for 21.0%(8/) 38),3 cases of sarcoma,1 case of metastasis,2 cases of adenoid adenoma,1 case of cavernous hemangioma,1 case of lymphoma.tumors have their own characteristics;tumor-like lesions accounted for 26.3%(10/38),tuberculosis 4 for example,1 case of abscess,3 cases of chronic inflammation,1 case of granuloma,clinical manifestations,involving the epididymis and skin,1 case of hematoma,history of trauma.Conclusion There are many kinds of testicular tumors and tumor-like lesions.Imaging is of great value as an auxiliary examination.Its CT/MR performance has certain characteristics.Clinical symptoms,age,and related laboratory tests can help diagnosis and differential diagnosis.
