摘　　要：目的恶性肿瘤发病与死亡的相关研究较多,但国内结合多年数据分析并计算疾病负担的研究较少。本研究通过分析2012-2014年江苏省江阴市户籍居民恶性肿瘤的发病、死亡水平及寿命、工作损失情况,为全市恶性肿瘤防控工作提供科学依据。方法收集2012-01-01-2014-12-31江阴市恶性肿瘤的发病、死亡监测数据,分性别、年龄和癌种计算发病/死亡构成比、粗发病/死亡率、标化发病/死亡率、累积发病/死亡率和潜在寿命/工作损失年等指标,并分析顺位。结果 2012-2014年江阴市恶性肿瘤的粗发病率、中标率和世标率分别为341.45/10万、192.68/10万和188.61/10万;粗死亡率、中标率和世标率分别为222.53/10万、116.89/10万和115.09/10万。男性发病和死亡水平均高于女性,男性、女性发病和死亡率均随年龄增长而上升。发病率较高的主要为胃癌、肺癌、结直肠肛门癌、肝癌、食管癌;死亡率较高的主要为肺癌、胃癌、肝癌、食管癌、结直肠肛门癌;寿命和工作损失较高的为肺癌、肝癌和胃癌。结论肺癌、消化系统恶性肿瘤及前列腺癌、女性乳腺癌的发病、死亡以及寿命损失水平普遍较高,是江阴市恶性肿瘤防控工作中应重点关注的癌种。此外,应加强对乳腺癌、白血病、脑及神经系统恶性肿瘤等的早期诊断和防治。OBJECTIVE While cancer incidence and mortality ars well-investigated all over the country,there are few studies based on years of data analysis and disease burden in China.This study was to analyze the incidence,mortality and life lost of cancer in Jiangyin,from 2012 to 2014.METHODS The data of cancer incidence and mortality of Jiangyin residents from 2012-2014 were collected and evaluated.Cancer incidence and mortality crude rate,age-standardized rate,cumulative rate,constituent ratioand proportion,life and work lost of top 10 cancers were calculated.RESULTS The crudeincidence of cancer in Jiangyin from 2012-2014 was 341.45/105,the age-standardized incidence rate by Chinese population was 192.68/105,the age-standardized incidence rate by world population was 188.61/105.The crude mortality of cancer in Jiangyin was 222.53/105,the age-standardized mortality rate by Chinese population was 116.89/105,the agestandardized mortality rate by world population was 115.09/105.The incidence and mortality in males were higher than those in females.Top 5incidence cancers were stomach,lung,colorectal,liver and esophagus cancers,top 5cancer deaths were lung,stomach,liver,esophagus and colorectal cancers,and top 3cancer of life and work lost were lung,liver,stomach cancers.CONCLUSION For both sexes,digestive system cancer and lung cancer are main cancers in Jiangyin,and attention should be paid to leukemia,Brain,CNS and breast cancers.
