摘 要:目的:采用meta分析评估ERCC1表达水平与鼻咽癌患者临床病理特征的关系。方法:采用计算机检索中国知网、万方数据库、pubmed、Web of science、Medline数据库中ERCC1与鼻咽癌关系的相关中英文文献,检索日期截止至2017年9月30日,提取符合纳入标准的文献中的相关资料,相对危险度(risk ratio,RR)和风险比(hazard ratio,HR)作为合并效应量,采用Stata 12.0软件进行meta分析。结果:共纳入18项研究,包括2 209例鼻咽癌患者,8项研究为ERCC1表达水平阳/阴性二分类对比,10项研究为ERCC1表达水平高/低二分类比较;meta分析结果显示:ERCC1蛋白阳性的鼻咽癌患者比阴性患者的总生存率低(Positive vs Negative,HR=2.618,95%CI:1.657~4.136,P=0.000);ERCC1相对高表达组鼻咽癌患者比低表达组的总生存率低(High vs Low,HR=1.812,95%CI:1.276~2.573,P=0.001);ERCC1高表达鼻咽癌患者组的无病生存率比低表达患者组低(High vs Low,HR=1.566,95%CI:1.395~1.758,P=0.000);ERCC1表达水平与鼻咽癌浸润、淋巴结转移、性别、年龄等其他临床病理特征的相关性差异无统计学意义。结论:ERCC1阳性/高表达鼻咽癌患者的生存预后差,ERCC1有望成为鼻咽癌患者个体化治疗的预后预测因子。Objective:To evaluate the relationship between expression level of ERCC1 and clinicopathological characteristics of patients with nasopharyngeal cancer (NPC).Method :Chinese or English articles relevant to NPC and ERCC1were retrieved from CNKI,Wanfang Data,Pubmed,Web of science,Medline Database by the end of September 2017. Relevant data were extracted from eligible studies based on inclusion criteria.RR(risk ratio)and HR (hazard ratio)were considered as pooled effect size.This meta-analysis was performed by using Stata 12.0.Results:A total of 18 studies in-volving 2209 patients with NPC were selected.Among those,dichotomous expression based on ERCC1^+or ERCC1-was discussed in 8 studies,while high or low level of ERCC1 was discussed in 10 studies.The resuhs of meta-analysis indicated that ERCC1^+patients were associated with poorer OS (HR =2.618,95%CI:1.657-4.136,P =0.000)compared with ERCC1^-patients,and patients in high ERCC1 level group was related to poorer OS (HR =1.812,95%CI:1.276N 2. 573,P =0.001)and DFS (HR =1.566,95%CI:1.395-1.758,P =0.000)compared with those in low ERCC1 level group.In addition,no statistically significant difference was observed among patients with different ERCC1 levels in clinico- pathological characteristics such as tumor invasion,lymph node metastasis,sex,and age etc.Conclusion:ERCC1^+NPC patients or NPC patients with high ERCC1 level would have a poor prognosis.ERCC1 is projected to become a predictive and prognostic factor for the individualized treatment of NPC.
