摘  要:目的评价SPECT/CT融合显像在全身骨显像中不典型病变中的临床诊断价值。方法通过对99mTc-MDP全身骨显像中117例共182个不典型病灶进行融合显像鉴别病变的性质,并结合最终临床诊断,统计分析单纯同机SPECT、CT和SPECT/CT的诊断效能。结果 SPECT/CT融合显像诊断117例共182个不典型病灶中恶性病灶83个,良性病灶99个,灵敏度91. 4%(74/81)、特异性90. 1%(91/101)、准确性90. 7%(165/182),与同机SPECT、CT诊断准确性相比较差异有统计学意义(P <0. 05)。结论 SPECT/CT融合显像可以明显提高全身骨显像中不典型病变灶诊断的准确率。Objective To evaluate the diagnostic value of SPECT/CT fusion imaging in atypical bone lesions. Methods One hundred and eighty-two atypical lesions of 117 patients by99mTc-MDP whole body bone imaging were performed SPECT/CT fusion imaging to identify malignant lesions from benign lesions. All results were compared with final clinical findings. The diagnositic efficacy indicators of SPECT,CT and SPECT/CT fusion imaging were compared and analyzed. Results Among 182 atypical lesions,83 were malignant lesions and 99 were benign lesions. The sensitivity,specificity and accuracy of SPECT/CT fusion imaging were 91. 4%( 74/81),90. 1%( 91/101) and 90. 7%( 165/182),respectively. Significant difference was found between the accuracy of SPECT/CT an single SPECT or CT( P < 0. 05). Conclusion SPECT/CT fusion imaging may improve the accuracy for diagnosing atypical bone lesions.
