奥曲肽联合雷贝拉唑治疗老年急性非静脉曲张性上消化道大出血56例临床分析

摘　　要：目的:总结奥曲肽联合雷贝拉唑对老年急性非静脉曲张性上消化道大出血的临床效果及安全性。方法:回顾性分析2015年3月-2017年12月广东省东莞市大朗医院和中山大学孙逸仙纪念医院急诊科收治的老年急性非静脉曲张性上消化道大出血患者124例的临床资料,将应用雷贝拉唑治疗的68例为对照组,在对照组治疗基础上加用奥曲肽的56例为观察组,比较两组24 h、48 h及72 h止血情况、临床疗效和药物不良反应。结果:观察组24 h止血率明显高于对照组(46.3%vs 35.3%,P<0.05),临床疗效明显优于对照组(96.4%vs86.8%,P<0.05),总药物不良发生率虽略高于对照组,但无明显差异(10.7%vs 7.4%,P>0.05)。结论:奥曲肽联合雷贝拉唑治疗老年急性非静脉曲张性上消化道大出血的临床疗效好,安全性高,建议临床推广应用。Objective:To summarize the clinical effect and safety on elderly patients with acute non-variceal uppergastrointestinal massive bleeding treated by Octreotide and Rabeprazole. Methods:The clinical data of 124 elderly pa-tients with acute non-variceal upper gastrointestinal massive bleeding were analyzed retrospectively from Mar 2015 toDec 2017 in the department of emergency of the two hospitals.68 cases treated by Rabeprazole were classified as controlgroup,and 56 cases treated Octreotide on the basic of the control group were classified as observation group. The stopbleeding rate at 24 hs,48 hs,72 hs and clinical effect, drug side effects were compared between the two groups. Results:The stop bleeding rate at 24 hs in the observation group was significantly higher than that in the control group(46.3% vs35.3%,P<0.05). The clinical effect in the observation group was significantly higher than that in the control group(96.4% vs 86.8%,P<0.05). Although the total side effects in the observation group were high than that in the controlgroup,but it was no significantly different(10.7% vs 7.4%,P>0.05). Conclusions:It is suggested that it should be popu-larized and applied in clinic for good clinical curative effect and high safety on elderly patients with acute non-varicealupper gastrointestinal massive bleeding treated by Octreotide and Rabeprazole.
