经皮肝穿刺球囊扩张十二指肠乳头治疗胆总管结石的疗效观察

摘　　要：目的 探讨经皮肝穿刺胆道球囊扩张十二指肠乳头取石术治疗胆道结石的安全性及可行性。 方法 对26例行DSA下十二指肠乳头扩张取石术胆总管结石患者的资料进行分析,分析该术式的成功率及并发症发生率。 结果 2例第1次手术时失败,引流1周后成功取石,其余24例均1次成功取石,成功率100%。术后3例患者出现不同程度的腹痛,对症处理后好转,未出现发热、胆道出血、术后胰腺炎等并发症,疼痛发生率为11.5%。 结论 DSA下十二指肠乳头扩张取石术治疗胆总管结石是相对安全、可靠、有效的方法,值得临床推广。Objective To evaluate the safety and feasibility of percutaneous transhepatic balloon dilation of duodenal papilla sphincter for common bile duct stones.Methods 26 patients who underwent percutaneous transhepatic balloon dilation of duodenal papilla sphincter were analyzed.Analyzing the success rate of the procedure and the incidence of complicanons.Results 24 patients were successfully stoned at the first time,2 cases which raided at first time were removed successfully after 1 week of drainage. Hence,the success rate of this method was 100%.Different degrees of abdominal pain happened in 3 patients after operation,but symptoms improved after treatment.No complications such as fever,biliary tract bleeding and postoperative pancreatitis occurred.The complication rate was 11.5%.Conclusion For the treatment of biliary ductatones,percutaneous transhepatic balloon dilation of duodenal papilla sphincter is safe,reliable and effective;This technique is worthy of clinical promotion.
