自噬蛋白ARHI/Beclin1与宫颈癌临床病理特征及预后的关系

摘　　要：目的分析自噬蛋白ARHI/Beclin1与宫颈癌临床病理特征及预后关系。方法选取2011年3月至2014年3月本院收治的13例宫颈良性病变患者、32例宫颈上皮内瘤变(CIN)患者和54例宫颈癌患者作为研究对象。采用免疫组化法检测各组织中自噬蛋白ARHI和Beclin1的表达情况,并分析其与宫颈癌临床病理特征的相关性;同时进行随访(截止日期2017年3月),记录并分析自噬蛋白ARHI和Beclin1与宫颈癌患者预后的关系。结果自噬蛋白ARHI和Beclin1在宫颈良性病变组、CIN组和宫颈癌组的阳性表达率依次降低,分别为84.62%、53.13%、27.78%和92.31%、62.50%、29.63%,三组间比较差异均有统计学意义(P<0.05);自噬蛋白ARHI与Beclin1呈正相关,在低分化、淋巴结转移及临床分期(Ⅲ～Ⅳ期)宫颈癌中阳性表达率低,且差异均有统计学意义(P<0.05);截止2017年3月,自噬蛋白ARHI和Beclin1阳性表达宫颈癌中生存率分别为66.67%、68.75%,高于阴性表达宫颈癌生存率(23.08%、26.32%),差异有统计学意义(P<0.05)。结论自噬蛋白ARHI与Beclin1在宫颈癌中呈正相关,且恶性程度越高的宫颈癌其阳性表达率越低,生存时间越短。Objective To discuss the relationship between autophagy protein ARHI/Beclinl and clinicopathological features/prognosis of cervical cancer. Methods 13 cases of cervix benign lesions, 32 cases of cervical intraepithelial neoplasia and54 cases of cervical cancer treated in Anyang tumor hospital from March 2011 to March 2014 were selected as the subjects.The expression of autophagy protein ARHI and Belinl were detected by immunohistochemistry in tissues, and was analyzed its correlation with cliniopathological features of cervical cancer. Follow-up,was also performed(ending date March 2017),and the relationship between autophagy proteins ARHI and Beclinl and survival outcomes in patients with cervical cancer was recorded and analyzed. Results The positive expression rates of autophagy proteins ARHI and Beclinl in benign cervical lesions, CIN group and cervical cancer group were 84.62%, 53.13%, 27.78% and 92.31%, 62.50%, 29.63%,respectively. The differences were statistically significant(P<0.05). The autophagy protein ARHI was positively correlated with Beclinl, and the positive expression rate was low in poorly differentiated, lymph node metastasis and clinical stage(stage Ⅲ~Ⅳ) cervical cancer,and the difference was statistically significant(P<0.05). As of March 2017, the survival rates of autophagy proteins ARHI and Beclinl positively expressed cervical cancer were 66.67% and 68.75%, respectively,which was higher than the survival rates of ARHI and Beclinl non expressed cervical cancer(23.08%, 26.32%). The difference was statistically significant(P<0.05). Conclusion The autophagy protein ARHI is positively correlated with Beclinl in cervical cancer, and the higher the degree of malignancy, the lower the positive expression rate of cervical cancer, and the shorter the survival time.
