摘 要:目的了解苏州地区医务人员心脏急救能力掌握情况,评价苏州市医务人员心肺复苏(cardiopulmonary resuscitation,CPR)理论和操作技能。方法对苏州市3家二级医院和15家社区卫生服务中心全部医护人员进行考核。评估医务人员生命支持理论中症状识别、胸外按压、人工通气、除颤措施、药物使用五个维度(共15题)答题正确率;使用CPR操作实时反馈系统实时监测按压深度、按压频率、充分回弹程度,计算得分(满分100分),评估医务人员CPR操作能力。结果共1 014名医务人员参与考核。53.3%在半年内接受过CPR培训。CPR技能知识中位数得分为10(8,12)分(满分15分),其中按压频率(84.3%)、按压通气比(93.2%)回答正确率较高,按压中断时间(46.0%)、球囊通气频率(29.8%)等问题正确率低。CPR操作中位数得分为85.00(32.75,98.00)分,平均按压频率126(118,143)次/min,平均按压深度5.9(5.2,6.5)cm,31%医护人员按压回弹不充分。半年内接受过培训者(53%)理论[10(8,12)分vs 9(7,11)分]和操作[90(42.5,98)分vs 59(19,92.5)分]表现均显著优于未接受培训者(P<0.001)。结论应建立心肺复苏常规培训、考核及复核体制,进一步加强基层医务人员心脏急救能力。Objective To investigate the cardiopulmonary resuscitation (CPR)awareness and capability for medical staff in Suzhou city.Methods Assessment was taken by all medical staff from 3 second-class hospitals and 15 community health service centers in Suzhou City.Evaluate the correctness of the five dimensions (a total of 15 questions)of symptom recognition,chest compressions,artificial ventilation,defibrillation measures,and ,drug use in medical personnel's life support theory;real-time monitoring of compression depth,compression frequency,and full recoiling using CPR operation real-time feedback system,calculation of score (out of 100 points).Results A total of 1014 medical personnel participated in the assessment.53.3% received CPR training within six months.The median score of CPR skill knowledge was 10 points (out of 15 points).The rate of correct response was high with compression frequency (84.3%),and compression/ventilation ratio (93.2%).The correct rate of problems such as compression interruption time (46.0%)and balloon ventilation rate (29.8%)was low.The median CPR operation score was 85.00 (32.75,98.00)points.The average compression rate was 126 (118,143)beats/min,and the average compression depth was 5.9 (5.2,6.5)cm.31% of the medical staff did not have enough pressure to recoiling.There was a significant difference between trained and untrained stuff in theoretical and operational performance (P <0.001).Conclusion A regular training,assessment and review system for CPR should be established to further strengthen a primary medical staff's ability to provide first aid to the cardiac emergency.
