腔镜带蒂大网膜获取技术在乳腺癌腔镜保乳肿瘤整形手术中的应用

摘　　要：目的研究腔镜带蒂大网膜获取技术在乳腺癌腔镜保乳肿瘤整形手术中的应用价值。方法回顾性分析2016年11月-2017年11月在首都医科大学附属北京友谊医院行腔镜保乳手术+腔镜带蒂大网膜肿瘤整形手术的11例乳腺癌患者的病例资料。记录并分析手术时间、术中出血量、拔除引流管时间、切口长度、手术并发症、术后美容效果、肿瘤复发情况。结果11例患者手术时间平均186min,术中出血量平均22ml,拔除引流管时间平均3.1d,切口总长度平均5.2cm。无严重并发症发生,1例患者出现术后嗳气增多。术后美容效果评分平均为14.7分(满分15分);其中“乳房外形”“切口瘢痕”2项,全部患者均为满分。元局部复发或远处转移病例。结论乳腺癌腔镜保乳手术与腔镜带蒂大网膜获取技术结合能安全而有效地恢复保乳手术后的乳房外形,达到较好的术后美容效果。Objective To explore the application value of laparoscopic pedieled omentum acquisition technique in laparoseopie breast-conserving surgery of breast eareinoma.Methods From November 2016 to November 2017,there were 11 patients with breast carcinoma underwent laparoseopie breast-conserving surgery and breast plastic surgery with laparoscopie pedicled omentum acquisition technique in Beijing Friendship Hospital,Capital Medieal University,and retrospectivly analyzed the clinical data of these patients.The operation time,intraoperative blood loss,drainage tube time,length of incision,surgical complications,postoperative cosmetic effect and tumor recurrence were recorded and analyzed.Results In 11 patients,the average of the total operation time was 186 rain,the average intraoperative blood loss was 22 ml,the average drainage tube time was 3.1 d,and the average total length of the incision was 5.2 cm.There was no severe complication after surgery but only one patient had an increase of belching.The postoperative cosmetic results were excellent with the mean score was 14.7 (the total score was 15)along with getting full score in "shape of breast"and "scar of the incision".There was no locoregional recurrence or distant metastases.Conclusion The combination of laparoscopic breast-conserving surgery of breast carcinoma and the laparoscopic pedicled omentmn acquisition technique assure the recovery of the breast shape safely and effectively,also provide better postoperative cosmetic results.
