1%吡美莫司乳膏治疗2岁以下婴幼儿特应性皮炎的临床研究

摘　　要：目的:观察1%吡美莫司乳膏治疗2岁以下婴幼儿特应性皮炎(AD)的临床效果。方法:2010年4—2016年5月确诊AD患儿68例,根据月龄不同分为3～12月龄试验组和对照组各18例;12～24月龄试验组和对照组各16例。疗程为4周,随访8周。试验组1%吡美莫司乳膏,每日2次外用,疗程结束后均以每周2次维持治疗,直至随访期结束;对照组0.1%丁酸氢化可的松乳膏,每日2次外用。结果:3～12月龄与12～24月龄的试验组1、2、4周与基线相比,湿疹面积及严重度指数(EASI)、瘙痒严重程度评分均明显降低,与对照组比较降低的更明显,差异均有统计学意义(P<0.05);随访期间,4组均未见反跳发生,不良反应率低。结论:外用1%吡美莫司乳膏治疗2岁以下AD患儿效果佳,疗效较为持续,值得推广。Objective: To investigate the clinical efficacy of 1% pimecrolimus cream in the treatment of atopic dermatitis(AD)in infants under 2 years of age. Methods: From April 2010 to May 2016, 68 children with AD were diagnosed in our hospital. The patients were divided into experimental group and control group(3-12 month old, 18 cases each;12-24 month old, 16 cases each). The experimental groups and controls were treated with topical 1% pimecrolimus and glucocorticoids, respectively, twice-daily at 12 hours interval, for 4 weeks, followed by twice weekly for 4 weeks. Results: In comparison to the controls, significant reductions in both scores of EASI and the severity of pruritus were observed in the experimental groups after 1, 2 and 4 weeks of treatments(P<0.05). 1% pimecrolimus cream could rapidly and effectively improve AD. The rebound rates were significantly lower in the experimental groups than in the controls at 1, 2 and 4 weeks of treatments(χ^2=16.355, P<0.001). No relapse occurred in all groups during the follow-up period. Conclusion: Topical 1% pimecrolimus cream is effective for AD in children under 2 years old, with rapid and sustained effect.
