腹腔镜治疗肥胖妇女早期子宫内膜癌的可行性分析

摘　　要：目的探讨腹腔镜治疗肥胖妇女早期子宫内膜癌的可行性。方法选取2015年1月至2017年12月间上海市闵行区中心医院收治的60例肥胖型早期子宫内膜癌患者为研究对象,将采用腹腔镜治疗的30例患者纳入观察组,采用传统开腹治疗的30例患者纳入对照组。比较两组患者治疗前后免疫功能指标、手术相关指标及并发症发生情况。结果观察组患者治疗后免疫球蛋白G(Ig G)和CD+4/CD+8指标均优于对照组患者,差异均有统计学意义(均P <0. 05)。观察组患者术中出血量、肛门排气时间及术后住院时间均低于对照组患者,差异均有统计学意义(均P <0. 05)。观察组术后并发症发生率为10. 0%,对照组患者为16. 7%,组间比较,差异无统计学意义(P> 0. 05)。结论腹腔镜手术治疗肥胖妇女早期子宫内膜癌,患者术中出血量少,术后恢复快,免疫功能恢复快,安全性高,值得推广应用。Objective To explore the feasibility of laparoscopy in the treatment of early endometrial carcinoma in obese women. Methods Sixty obese women with early endometrial cancer who were treated at Central Hospital of Minxing District from January 2015 to December 2017 were selected as the subjects. The30 patients who underwent laparoscopic surgery were included in the observation group and 30 patients who underwent traditional open abdominal surgery were included in the control group. The immune function indexes,operation related indexes and the incidence of complications were compared between the two groups.Results The levels of immunoglobulin G( Ig G) and CD4^+/CD8^+ were better in the observation group than in the control group( all P < 0. 05). The amount of bleeding,time to anus exhaust and time of hospitalization were lower in the observation group than in the control group( all P < 0. 05). The incidence of complications was 10. 0% for the observation group and 16. 7% for the control group,but the difference was not statistically significant( P > 0. 05). Conclusion Laparoscopic operation for the treatment of early endometrial carcinoma in obese women is less bleeding. It is beneficial to the recovery of the patients after operation. The immune function of the patients is improved and the safety is high. It is worth popularizing.
