摘 要:目的探讨miR-155在乳腺癌患者组织和血浆中的表达水平及临床意义,为乳腺癌早期诊断和治疗及预后分析提供参考依据。方法收集38例首次确诊乳腺癌患者癌组织标本、正常组织、血浆标本及健康对照组,应用FQ-PCR方法检测乳腺癌患者组织和血浆中miR-155的表达,同时采用化学发光法检测血清CA153水平。分析miR-155表达水平与临床病理资料的关系,应用受试者工作曲线(ROC曲线),并与CA153水平进行比较,判断miR-155在乳腺癌诊治中的价值。结果在乳腺癌患者组织和血浆中,miR-155的表达显著高于正常对照组,差异有统计学意义(均P<0. 01),miR-155的表达水平与乳腺癌患者肿瘤大小、淋巴结转移、雌激素受体/孕激素受体(ER/PR)及人类表皮生长因子受体-2 (Her-2)的表达等因素显著相关,差异有统计学意义(P<0. 05); ROC曲线分析表明,组织miR-155曲线下面积(AUC)是0. 929,特异度78. 9%,敏感度89. 5%,cut-off值-2. 426;血浆miR-155的AUC为0. 882,特异度77. 3%,敏感度86. 8%,cut-off值-2. 739。乳腺癌患者血清CA153的水平与健康对照组、乳腺良性疾病组以及术前和术后比较,差异无统计学意义(均P> 0. 05),且总体均值<25 ng/ml。结论 miR-155在乳腺癌诊断、疗效监测和预后评估具有潜在的应用价值,是乳腺癌可行的生物标志物。Objective To explore the expression level and clinical significance of miR-155 in tumor tissue and plasma of cervical cancer patients,provide a reference basis for early diagnosis,treatment,and prognosis analysis of cervical cancer. Methods The tumor tissue,normal tissue,and plasma samples of 38 breast cancer patients diagnosed definitely for the first time were collected,healthy tissue samples were obtained during the same time as control group. The expression levels of miR-155 in tumor tissue and plasma were detected by FQ-PCR. Chemiluminescence method was used to detect CA153 levels in serum. The relationship between miR-155 expression level and clinicopathological data was analyzed. ROC curve was used to evaluate the value of miR-155 in diagnosis and treatment of breast cancer.Results The expression levels of miR-155 in tumor tissue and plasma of breast cancer patients were statistically significantly higher than those in normal control group (P<0. 01). The expression level of miR-155 was significantly correlated with tumor size,lymph node metastasis,ER/PR,and Her2 expression in breast cancer patients (P < 0. 05). ROC curve analysis showed that AUC of tissue miR-155 was 0. 929,the specificity was 78. 9%,the sensitivity was 89. 5%,and the cut-off value was-2. 426. AUC of plasma miR-155 was 0. 882,the specificity was 77. 3%,the sensitivity was 86. 8%,and the cut-off value was-2. 739. There was no statistically significant difference in the level of serum CA153 between breast cancer patients and normal control group,benign breast disease group,and there was no statistically significant difference in the level of serum CA153 between before and after operation in breast cancer patients (P>0. 05). The total mean value was less than 25 ng/ml.. Conclusion miR-155 has potential application value in diagnosis,effectiveness monitoring,and prognostic evaluation,which is a feasible biomarker of breast cancer.
