摘 要:目的研究多系统萎缩(MSA)患者肝肾功能、血脂、同型半胱氨酸水平的变化,探讨血清生化指标水平与MSA发病及疾病严重程度的关系。方法回顾性选择2014年1月至2017年6月期间在河南省人民医院神经内科住院治疗的86例多系统萎缩患者,选择同期诊治的85例健康人为对照组,比较两组患者血脂、肝、肾功能等指标;进行性别分层,评估肝肾功能、血脂、同型半胱氨酸水平与MSA发病和严重程度的相关性。结果MSA组的总胆固醇、高密度脂蛋白胆固醇和低密度脂蛋白胆固醇水平低于对照组(均P<0.01)。男性MSA患者总胆固醇、高密度脂蛋白胆固醇和低密度脂蛋白胆固醇水平低于对照组(P<0.01);女性MSA患者尿酸水平低于对照组(P<0.05)。年龄小于60岁的MSA患者总胆固醇和低密度脂蛋白胆固醇水平低于健康对照组(P<0.01);MSA患病与总胆固醇(r=0.34,P<0.01)、高密度脂蛋白胆固醇(r=-0.27,P<0.01)、低密度脂蛋白胆固醇(r=0.32,P<0.01)和尿素(r=0.15,P<0.05)相关,但不是MSA患病的独立危险因素。总胆固醇(r=0.27,P<0.05)和低密度脂蛋白胆固醇(r=0.23,P<0.05)水平与MSA严重程度相关,但均非MSA严重程度的独立危险因素(r=-0.381,P>0.05)。结论虽然血脂、尿素水平与MSA患病相关,但均不是MSA患病的独立危险因素,亦不是MSA病情严重程度的危险因素。性别可能影响MSA患者发病,但尚需进一步证实。ObjectiveTo study changes in hepatic and renal function, serum lipids and homocysteine levels in patients with multiple system atrophy(MSA), and to explore the relationship of serum biochemical indexes with MSA and its severity. MethodsA total of 86 hospitalized patients with MSA at our department from January 2014 to June 2017 were retrospectively analyzed.Eighty-five healthy subjects undergoing health physical examinations at our hospital during the same period were enrolled as a control group.Serum lipid levels and liver and renal function were compared between the two groups.After stratified by gender, the correlations of liver and renal function, serum lipids and homocysteine levels with MSA and its severity were analyzed. ResultsSerum levels of total cholesterol, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol were lower in the MSA group than in the control group(P<0.01). Serum levels of total cholesterol, high-density lipoprotein cholesterol and low-density lipoprotein cholesterol were lower in male patients with MSA than in male healthy controls(P<0.01). Serum levels of uric acid were lower in female patients with MSA than in female healthy controls(P<0.05). Serum levels of total cholesterol and low-density lipoprotein cholesterol were lower in MSA patients aged under 60 years than in healthy controls aged under 60 years(P<0.01). Serum levels of total cholesterol(r=0.34, P<0.01), high-density lipoprotein cholesterol(r=-0.27, P<0.01), low-density lipoprotein cholesterol(r=0.32, P<0.01)and uric acid(r=0.15, P<0.05)were correlated with MSA, but they were not independent risk factors.Serum levels of total cholesterol(r=0.27, P<0.05)and low-density lipoprotein cholesterol(r=0.23, P<0.05)were correlated with the severity of MSA, but they were not independent risk factors(r=-0.381, P>0.05).
