糖皮质激素在重度溃疡性结肠炎治疗中的应用效果分析

摘　　要：为探讨糖皮质激素在重度溃疡性结肠炎(UC)治疗中的应用效果,将2015年1月至2018年1月我科收治的100例重度UC患者随机分为对照组和观察组,各50例,对照组常规口服柳氮磺吡啶肠溶片治疗,观察组在对照组治疗基础上,予二丙酸倍氯米松3 mg和生理盐水250 mL的混合液保留灌肠治疗。比较2组临床疗效、血清炎症因子指标、免疫功能指标及不良反应发生情况。结果显示,1)临床疗效:观察组总有效率明显高于对照组,P <0.05。2)血清炎症因子:治疗后,观察组C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)水平均低于对照组,P <0.05。3)免疫功能指标:治疗后,观察组CD3^+、CD4/CD8等免疫功能指标均高于对照组,P <0.05。4)不良反应:2组不良反应发生率比较差异无统计学意义,P>0.05。结果表明,糖皮质激素保留灌肠用于治疗重度UC,不仅可有效抑制患者机体炎症反应,改善其免疫功能,提高临床疗效,而且不会增加不良反应,安全可靠。To investigate the application efficacy of glucocorticoid in the treatment of severe ulcerative colitis(UC)100 cases of severe UC who received treatment in author's dept.(2015-01-2018-01)were randomly divided into control group (n=50,routinely orally administered salazosulfapyridine enteric-coated tablets)and observation one(n=50,also added retention enema with 3 mg of beclomethasone dipropionate and 250 mL of normal saline),then both groups' clinical effect,serum inflammation factor indexes, immunity function indexes and adverse reaction incidence were compared.As results,1)clinical effect,the total effective rate of observation group was significantly higher than that of control one(P<0.05);2)serum inflammation factors.after treatment in observation group the levels of CRP,IL-6 and PCT were all lower than in control one(P<0.05);3)immuno-function indexes,after treatment in observation group CD4/CD8,etc indexes were all higher than in control one(P<0.05);4)adverse reaction:there was no statistical difference between the two groups(P<0.05).Results show that retention enema with glueocorticoid for severe UC not only can inhibit body's inflammatory response,improve its immuno-function,enhance clinical effect,but do not increase incidence of adverse reaction,thus it is safe and reliable.
