摘 要:目的探讨前方牵引器在生长发育高峰期后骨性安氏Ⅲ类错患者中的应用效果。方法选择沈阳市口腔医院2013年3月至2018年3月收治的生长发育高峰期后骨性安氏Ⅲ类错患者30例,其中15例采用前方牵引联合基托式扩弓器快速扩弓配合固定矫治,15例采用单纯固定矫治,对矫治前后硬组织测量指标SNA角(蝶鞍中心、鼻根点及上齿槽座点所构成的角)、SNB角(蝶鞍中心、鼻根点及下齿槽座点所构成的角)、ANB角(上齿槽座点、鼻根点与下齿槽座点构成的角)、U1-NA角(上中切牙长轴与鼻根点-上齿槽座点连线交角)、L1-NB角(下中切牙长轴与鼻根点-下齿槽座点连线的交角)、U1-L1角(上中切牙长轴与下中切牙长轴的交角)、NA-PA角(颌凸角)、MP-SN角(下颌平面与前颅底平面的交角)、Wits值、MP-FH角(下颌平面与眼耳平面的交角)、APDI(矢状向异常指数),软组织测量指标Gl'-Sn-Pog'角(面型角)、Cotg-Sn-Ls角(鼻唇角)等数据进行对比分析。结果所有患者面型均改善,两组间矫治前后NA-PA角、Wits值、APDI、Gl'-Sn-Pog'等指标的差值比较差异有统计学意义(P<0.05),前方牵引联合基托式扩弓器快速扩弓配合固定矫治组的改善优于单纯固定矫治组。结论前方牵引在改善患者的软组织侧貌美观方面优于单纯固定矫治,是一种临床相对有效的矫治骨性Ⅲ类错的方法。ObjectiveTo discuss the treatment effect of facemask protraction in the treatment of patients with skeletal Class Ⅲ malocclusion after growth peak.MethodsTotally 30 patients with skeletal Class Ⅲ malocclusion after growth peak were selected from March 2013 to March 2018 in Stomatology Hospital of Shenyang:15 patients were treated with anterior traction combined with fixed treatment,and 15 patients were treated with simple fixed treatment. The measurement indexes of hard tissue before and after treatment were compared and analyzed,including SNA,SNB,ANB,U1-NA,L1-NB,U1-L1,NA-PA,MP-SN,Wits value,MP-FH,APDI,Gl’-Sn-Pog’angle,and Cotg-Sn-Ls angle.ResultsAll patients had improved facial shape,and the difference in NA-PA,Wits value,APDI and Gl’-Sn-Pog’ had statistical significance(P < 0.05). The improvement of facemask protraction combined with rapid expansion of base expander and fixed orthodontic treatment was better than that of simple fixed orthodontic treatment.ConclusionFacemask protraction is a relatively effective method for the correction of skeletal Class Ⅲ malocclusion,which is superior to simple fixed orthodontics in improving the soft tissue profile of patients.
