摘　　要：目的分析2015-2017年重庆市沙坪坝区社区登记精神分裂症患者的死亡原因及特征,为进一步改进和完善精神分裂症患者社区健康管理服务提供依据。方法利用国家严重精神障碍信息系统,对2015年至2017年沙坪坝区登记的176例精神分裂症死亡患者的死亡原因和特征进行统计分析。结果沙坪坝区精神分裂症患者平均死亡年龄(63. 21±14. 97)岁,已婚者占48. 30%,小学文化程度占31. 82%,贫困患者占28. 98%,精神疾病病程平均为(24. 14±14. 25)年;死亡原因以躯体疾病为主(70. 45%),其后依次为其他原因(18. 75%),自杀(4. 50%),意外(3. 41%),精神病相关并发症(2. 80%);不同婚姻状况(χ~2=131. 31)、文化程度(χ~2=76. 80)、职业(χ~2=102. 57)、初次发病年龄(χ~2=96. 01)、精神疾病病程(χ~2=71. 51)之间死因构成差异有统计学意义(P均<0. 05)。结论重庆市沙坪坝区精神分裂症患者主要死因以躯体疾病为主,不同婚姻状况、文化程度、职业、初次发病年龄、精神疾病病程之间死因不同,应对不同人群采取不同的干预措施,特别应着力解决贫困患者医疗保障问题,努力提升健康管理服务质量,从而降低患者的死亡率。Objective To analyze causes and characteristics of death in schizophrenics in Shapingba district of Chongqing from 2015 to 2017 in order to provide a basis for further improvement of community health management services for schizophrenics. Methods The characteristics and causes of death of 176 registered patients with schizophrenia in Shapingba District were analyzed based on data from the national information system of psychosis using SAS8. 2. Results The average age of death of schizophrenic patients in Shapingba District was( 63. 21±14. 97) years, 48. 30% were married, 31. 82% were educated in primary schools,28. 98% were poor,and the average course of mental illness was( 24. 14 ± 14. 25) years;the main cause of death was physical diseases( 70. 45%),followed by other causes( 18. 75%),suicide( 4. 50%),accidents( 3. 41%) and mental illness related complications( 2. 80%). There were significant differences in the composition of causes of death among patients different in marital status( χ^2= 131. 31),educational background( χ^2= 76. 80),occupation( χ^2=102. 57),age of onset( χ^2= 96. 01) and course of mental illness( χ^2= 71. 51). Conclusion The registered patients with schizophrenia mainly die of physical illness and the death causes are different with respect to marital status,educational background,occupation,age of onset,and course of mental illness. Different interventions should be taken in various groups,and special efforts should be made to provide medical insurance for the poor so as to improving the quality of health management services and reducing the mortality.
