左旋卡尼汀改善维持性血透患者营养状况的作用

摘　　要：目的 :评价应用左旋卡尼汀治疗的维持性血透患者的营养状况。方法 :采用随机、对照的研究方法 ,选择维持性血透患者 2 0 0例 ,分为治疗组 (12 1例 )和对照组 (79例 ) ,每次透析结束后 ,治疗组静注左旋卡尼汀 1g,对照组静注生理盐水 2 ml,连续 3个月。 结果 :治疗组恶心、食欲减退、乏力等症状的发生程度和所占比例明显低于对照组 ,干体质量和上臂肌围值明显高于对照组 (P<0 .0 5 ) ,血红蛋白、血浆总蛋白、白蛋白、转铁蛋白含量显著升高 (P<0 .0 5 ) ,血浆游离左旋卡尼汀浓度明显高于对照组 (P<0 .0 1)。 结论Objective: To assess the nutritional status of m aintenance hem odialysis patients treated with L - carnitine. Methods:Two hundred maintenance hem odialysis patients were random ly divided into 2 groups:treatm entgroup(n=12 1) and control group(n=79) .In the treatm ent group,1g L - carnitine was given to the patients at the end of each dialysis treatm ent, the patients in the control group were given normal saline,and the treatment period was 3months. Serum total protein, albumin,free carnitine level,clinical and laboratory parameters were analysed. Results:Com pared with the control group, clinical sym ptoms of the treatm ent group were im proved,lean body weight and m id- arm m uscle circumference,hemoglobin, serum total protein,album in,transferrin,free L - carnitine level of the treatm ent group were elevated significantly (P <0 . 0 5 ) . Conclusion:Nutritional status of m aintenance hem odialysis patients treated with L - carnitine are better than those of patients without L - carnitine. [
