CYP1A1和GSTM1与胃癌、萎缩性胃炎和胃溃疡等易感性的研究

摘　　要：[目的]探讨细胞色素氧化酶基因 (CYP1A1)和谷胱甘肽转硫酶基因(GSTM1)多态性与胃癌和萎缩性胃炎等胃部疾病易感性的关系。[方法]对病理诊断的胃癌 (GC)102例、慢性萎缩性胃炎(CAG)110例、胃溃疡(GU)62例、慢性浅表性胃炎(CSG)103例、十二指肠溃疡(DU)62例、正常人62例的CYP1A1和GSTM1基因型采用序列特异性引物的聚合酶链反应(PCR SSP)方法进行测定,关联度分析采用病例对照研究方法。[结果]非条件Logistic回归在调整年龄、性别、文化程度和职业4个因素后,GC、CAG和GU与CYP1A1G/G、GSTM10/0基因型、幽门螺杆菌 (Hp)感染及吸烟有关联 ,同时基因型间存在明显的交互作用。没有发现DU和CSG与CYP1A1和GSTM1基因型有关联 ,但DU与幽门螺杆菌 (Hp)感染有关联 ,而且Hp感染、吸烟与CYP1A1G/G型之间存在交互作用。[结论]CYP1A1G/G、GSTM10/0基因型与GC、CAG、GU的易感性有关联 ,两个基因型之间及它们各自与Hp感染及吸烟之间有交互作用。To explore the relationship between CYP1A1,GSTM1 gene polymorphism and susceptibility to gastric cancer(GC),chronic atrophic gastritis(CAG),gastric ulcer(GU),duodenal ulcer(DU) and chronic superficial gastritis (CSG) in Chinese.GC 102 cases,CAG 110 cases,GU 62 cases,CSG 103 cases and DU 62 cases,which had been diagnosed by histopathology,and 62 healthy persons were analyzed by polymerase chain reaction of sequence specific primers(PCR SSP).The approach of case control study was used in the analyses of risk factors.In the model of non conditional Logistic regression analyses after adjusted age,sex,educational levels and profession,the risk factors of GC,CGA and GU were CYP1A1 G/G and GSTM1 0/0 genotypes,and Helicobacter Pylori(Hp) infection,and smoking.There were an interactive effects between exposure either to CYP1A1 G/G genotype and GSTM1 0/0 genotype or between one of the two genotypes and Hp infection or smoking.No close relationship between CSG or DU and the two genotypes was found.However,Hp infection was found to be a risk factor for DU, and also there was an interactive effect between CYP1A1 G/G genotype and Hp infection or smoking.[Conclusion]The CYP1A1 G/G and GSTM1 0/0 genotypes is related to susceptibility of GC,CAG and GU.
