血清MG_7-IPCR诊断卵巢癌的价值研究

摘　　要：目的 :从分子水平探讨胃癌相关抗原 MG7- Ag对卵巢恶性肿瘤早期诊断的临床意义。方法 :应用免疫 -PCR(polymerase chain reaction)技术检测 74例卵巢肿瘤患者血清 MG7- Ag水平 ,并分析其与卵巢恶性肿瘤临床病理特征的关系。结果 :1血清 MG7- Ag免疫 - PCR技术诊断卵巢恶性肿瘤的敏感性为 6 9.1% ,特异性为 89.5 % ,准确性74.3% ,阳性预测值为 95 % ,阴性预测值为 5 0 %。 2粘液性肿瘤的 MG7- Ag阳性表达率为 6 3.6 % ,显著高于浆液性肿瘤 (31.3% )。 3该方法对早期粘液性癌有很高的诊断敏感性。 4血清 MG7- Ag阳性表达率在卵巢恶性肿瘤临床分期、病理分级、肿瘤大小及患者年龄之间无显著差异。结论 :血清 MG7- Ag免疫 - PCR技术对卵巢恶性肿瘤具有普遍诊断意义 ,可作为临床早期诊断卵巢癌的检测手段 。Objectives:To evaluate the clinical significance of MG 7-Ag for diagnosing early stage ovarian cancer from the point of molecule.Methods:Serum levels of MG 7-Ag were detected using an Immuno-PCR(Polymerase Chain Reaction)technique in 74 patients with ovarian neoplasm,then analysed the relation between MG 7-Ag levels and clinical pathological characteristic of ovarian cancer.Results:①The diagnostic sensitivity,specificity, accuracy,positive and negative predictive value of MG 7-Ag were69.1%,89.5%,74.3%,95% and 50% respectively.②The positive rate of MG 7-Ag for mucinous epithelial tumor was 63.6%,which was higher significantly than that for serous epithelial tumor(31.3%).③The method has a high diagnosing sencitivity for early stage mucimous cancer.④The expression of MG 7-Ag was not associated with the clinic stage,histologic grade,the size of tumor and the age.Conclusions:The detection of serum MG 7-Ag has diagnostic significant for ovarian cancer of different types.It can be used to diagnose ovarian cancer of early stage,especially applies to diagnose mucinous epithelial cancer.
