肛门外结直肠低位手工吻合治疗中低位直肠癌146例分析

摘　　要：目的探讨中低位直肠癌手工吻合的低位保肛手术方法。方法采用结、直肠肛门外低位手工吻合的方法 ,对 14 6例中低位直肠癌行保肛手术 ,直肠肿瘤远端及其系膜的切除范围为 3～ 6cm。结果手术成功 14 5例 ,出现吻合口瘘 11例 ,仅 1例行肠造口 ;2年内复发和转移 13例 ,其中吻合口复发 2例 ;术后肛门对成形粪便控制力良好 ,无大便完全失禁者。结论肛门外结、直肠低位手工吻合是中低位直肠癌行保肛手术既可靠 。Objective To explore the method of external coloproctostomy with reservation of anus for treatment of middle or lower rectal carcinoma.Methods 146 cases with middle or lower rectal carcinoma were underwent with external coloproctostomy with reservation of anus. The distal resection of rectal carcinoma were 3～6 cm from tumors.Results 145 cases were successful in which one case was performed with colostomy and 11 case suffered from anastomotic leakage. 13 cases were reoccurred and metastased within 2 years. The anus functioned well and no incontinence of faeces happened after surgery.Conclusion External coloproctostomy with reservation of anus is available and economic way for treatment of middle or lower rectal carcinoma.
