摘 要:目的 以彩色多普勒成像 (CDI)技术探讨老年黄斑变性 (AMD)眼血流灌注缺损。方法 应用 CDI技术检测非渗出型 AMD患者 30例 (6 0只眼 )的眼动脉 (OA) ,颞、鼻侧睫状后短动脉 (TPCA、NPCA)及视网膜中央动脉 (CRA)血流。动脉血流指标为 PSV、EDV及 RI。结果 非渗出型 AMD与正常对照组比较 ,CRA、TPCA、NPCA的血流速度均明显降低 (P <0 .0 1)。而眼动脉血流速度无明显变化 (P >0 .0 5 )。结论 (1)老年黄斑变性的鼻、颞侧睫状后短动脉的血流速度均降低 ,表明 AMD的脉络膜血流灌注不良 ;(2 )老年黄斑变性的视网膜中央动脉的血流速度降低 ,提示 AMD除脉络膜灌注不良外 。Objective To study ocular perfusion defects in nonexudative age-related macular degeneration.Methods Thirty subjects with nonexudative age-related macular degeneration(60 eyes) with 30 age-matched control subjects(60 eyes) in studies of flow velocities in several retrobulbar vessels(OA.TPCA.NPCA.CRA)by CDI.We measured PSV.EDV velocity in the ophthalmic.central retinal and nasal and temporal posterior ciliary arteries.RI were calculated from the peak systolic and end diastolic velocity.Results Compared with the control,the Nonexudative AMD patients showed a highly statistically significant(P<0.01) decrease in the velocities in TPCA.NPCA,CRA and the resistive index(RI) was significantly increased in the TPCA or NPCA and CRA.Subjects with nonexudative AMD did not differ from control subjects in PSV,EDV,or RI in the ophthalmic artery (P>0.05).Conclusions (1)Age-related macular degeneration show a reduced blood flow veloeities in TPCA and NPCA,suggesting that choroidal perfusion is abnormal(Perfusion defects);(2)The changes in the CRA suggest there may be a more generalized perfusion abnormality beyond the choroid in patients with AMD.
