CT鼻咽腔造影在鼻咽癌放疗后复查中的应用

摘　　要：目的　CT鼻咽腔造影于鼻咽癌放疗后复查病例 ,探讨放疗后鼻咽腔壁脓性分泌物、痂皮影像学表现。方法　 34例共 37次CT鼻咽腔造影 ,造影与平扫和鼻咽光纤镜检对照分析。结果　放疗后鼻咽腔壁脓性分泌物、痂皮CT影像较具特征性 ,平扫呈条片状软组织密度物 ,表面毛糙、条网状影 ,间杂小气泡。造影扫描条片状软组织密度物悬浮于鼻咽腔内 ,对比剂渗入痂皮与鼻咽壁之间 ,形成一细线状间隔。结论　CT鼻咽腔造影有利于认识放疗后脓性分泌物、痂皮CT表现 ,当与腔内复发灶鉴别困难时应用该方法有助于鉴别诊断。Objective To study the image manifestation of purulent excreta and crust on the nasopharyngeal wall in postradiotherapy cases by using nasopharyngographic CT scan. Methods Thirty seven times of nasopharyngographic CT scan were performed in 34 cases, and the findings were analyzed with those of the plain scanning and pharyngorhinoscopy. Results The purulent excreta and crust in postradiotherapy cases had their characteristics. CT plain scanning displayed some soft tissue stripped density foci, coarse surface, reseau image, with some small bubbles. Nasopharyngographic CT scan was manifested with soft tissue stripped density foci, which floated on nasopharynx. The contrast agent filtered between crust and nasopharyngeal wall and formed a linear interval. Conclusion Nasopharyngographic CT scan benefits to display the purulent excreta and curst image. This method is helpful for distinguishing purulent excreta and crust from recurrent foci.
