长春新碱卡铂甲氨蝶呤联合治疗滋养细胞肿瘤76例疗效分析

摘　　要：目的　观察长春新碱 (VCR)、卡铂 (CBP)、甲氨蝶呤 (MTX)联合治疗滋养细胞肿瘤的疗效及毒副反应。方法　分析 76例初治滋养细胞肿瘤患者 ,采用VCR、CBP、MTX联合化疗的近期疗效、毒副反应及随访结果。结果　 76例经治疗后全部治愈 ,近期临床治愈率为 10 0 % ,其hCG降至正常或达近期临床治愈所需的平均疗程数分别为 1 92个和 2 71个 ;经随访复发率为 1 3% (1/ 76 ) ,4 8例保留生育功能者已获妊娠 30例次 ,妊娠废胎率为 10 0 % ;主要的毒副反应为骨髓抑制 ,而消化道反应较轻。结论　VCR、CBP、MTX联合治疗滋养细胞肿瘤 ,具有疗效好 ,显效快 ,毒副反应轻等优点 ,是理想的联合化疗方案之一。Objective To observe the effectiveness,toxicity and side effects of combination chemotherapy by vincristine(VCR),carboplatin(CBP) and methotrexate(MTX) for the treatment of trophoblastic tumor.Methods 76 patients with trophoblastic tumor were treated with combination chemotherapy by VCR,CBP and MTX,and the short term effectiveness,side effects and long-term results were reviewed.Results The immediately complete remission rate of all the 76 patients were 100%.The median number of course needed for the immediate treatment and that of descending to normal level of hCG was 2 71 and 1 92,respectively.The recurrence rate of long-term follow-up was 1 3%(1/76).30 of the 48 cases remaining the fertility capability were pregnant,10% of them with dead or unhealthy fetus;The main toxicity and side effects of the combination proctocal was myelosuppression,and the gastrointestinal symptoms were relatively slight.Conclusion The combination regimen of VCR,CBPand MTX in the treatment of trophoblastic tumor is effective,rapid and with less toxicity.
