重性抑郁症的神经心理与血清皮质醇水平的相关研究

摘　　要：目的 :探讨重性抑郁症患者神经心理学和皮质醇浓度的相关性。方法 :对符合DSM -Ⅳ的 4 0例重性抑郁症 (MD)患者和 37例正常对照使用全套WAIS智力测验 (WAIS -RC)、WMS逻辑记忆与视觉再生测验、STROOP、词汇流畅性 (VF)、延线测验A ,B (TRAILSA ,B)、汉诺塔 (TOH)、威斯康辛卡片分类测验改良版(M -WCST)进行评测 ,同时进行血清皮质醇的浓度测定。结果 :MD存在认知功能损害 ,涉及双侧额叶、双侧颞叶边缘系统及右侧顶叶。血清皮质醇的升高在MD患者中与对照组的差异有显著性 ,主要和记忆损害相关。结论Objective:Using neuropsychological tests to assess the cognitive functions of major depression(MD)and analyze the relationship between the cognitive impairment and the alternation of serum cortisol level.Methods:40 patients with MD according to DSM-Ⅳ and 37 normal controls were tested by WAIS-RC,Logical Memory and Visual Reproduction subsets of WMS,Stroop Test,Verbal Fluency,Trails Making,Tower of Hanoi,and Modified version of Wisconsin Card Sorting Tests.Blood samples were adopted for the measurement of serum cortisol level.Results:MD have cognitive impairments reflecting the dysfunction in frontal,temporal,limbic system and parietal lobes.The significantly elevated concentration of cortisol in MD related with the amnesic impairment.Conclusion:Serum Cortisol level elevated is related to the cognitive dysfunction in MD.
