摘  要:目的:拓宽螺旋CT血管造影(CTA)检查范围,探索颅内脑膜瘤新的影像检查方式。方法:应用改进后CTA检查56例颅内脑膜瘤。将CTA注药方式由原来的单向单流程1次延迟,改为单向双流程2次延迟。然后,应用表面遮蔽法(SSD)、最大密度投影法(MIP)和多平面重组(MPR),3种软件对原始扫描数据进行图像的工作站后处理。结果:所有血供丰富的脑膜瘤均获得颅骨、血管、肿瘤同时显示的三维立体图像。血管重建显示脑膜瘤染色,及脑膜瘤与周围血管的关系。颅骨重建可示脑膜瘤对邻近颅骨的影响。结论:对颅内脑膜瘤的诊断,CTA在肿瘤的定性、定位、大小、外观血供等方面均能为临床提供有实用价值的诊断依据,并可为神经外科提供全面术前影像信息。Objective: To explore a new inspection method for the diagnosis of intracranial meningioma, and to expand the use of CTA. Methods: Improved examinations of CTA were performed in 56 patients with intracranial meningioma. The method of CTA perfusion with one-way an monocircuit for one time retardation changed to one-way and dualcircuit for two times retardation. All data were analyzed with the software of maximum intensity projection (MIP), shaded surface display (SSD), and multiplanner reconstruction (MPR). Results: The skull, tumor, and blood vessels were simultaneously present in spiral CT angiography in meningioma with rich blood supply. The blood vessel reconstruction showed a clear meningioma dyeing and relation between meningioma and its peripheral blood vessel. The skull reconstruction showed the influence of meningioma on the skull. Conclusion: CTA provides a practical value in the diagnosis of intracranial meningioma in qualitative, location, size, and blood supply. It can also offer the preoperative imaging data to neurosurgeon.
