胃癌组织中转化生长因子和血小板源生长因子表达的意义

摘　　要：目的:研究转化生长因子(TGF-β_1)、血小板源生长因子(PDGF-A)在胃癌中表达及其与预后的关系。 方法:应用免疫组织化学方法和分子原位杂交方法检测有随访资料的101例胃癌手术切除标本中的TGF-β_1蛋白、PDGF-A蛋白及mRNA表达。 结果:在101例胃癌手术切除标本中TGF-β_1阳性表达者48例(47.5％)。TGF-β_1表达阳性率:有淋巴结转移者明显高于无淋巴结转移者(x^2=4.4431,p<0.05);脉管内有癌栓者明显高于无癌栓者(x^2=9.8568 p<0.01);癌组织浸润程度:粘膜及粘膜下、肌层、浆膜下及浆膜,三者比较有显著性差异(x^2=7.4799,p<0.05);组织学分级之间比较有显著性差异(x^2=9.1160,p<0.05).PDGF-A阳性表达者43例(42.6％)。PDGF-A表达阳性率:有淋巴结转移者明显高于无淋巴结转移者(x^2=5.5702,p<0.05);脉管内有癌栓者明显高于无癌栓者(x^2=8.8080,p<0.01);癌组织浸润程度:粘膜及粘膜下、肌层、浆膜下及浆膜,三者比较有显著性差异(x^2=14.0504,p<0.01)。PDGF-A阳性表达者5a及10a生存率明显低于阴性表达者5a及10a生存率(p=0.0174,p=0.0038),说明PDGF-A阳性表达者预后较差,阴性表达者预后较好。PDGF-A mRNA表达(61.5％)与PDGF-A蛋白表达(42.6％)一致。PDGF-A阳性表达与TGF-β_1阳性表达之间无关(p>0.01),但他们阳性表达?AIM:To investigate the expression of TGF-β1 and PDGF-A in gastric cancer tissue (GC) and its correlation with the prognosis.METHODS: The immunohistochemical staining and in situ hybridization technique were used to detect the TGF-β protein, PDGF- A protein and mRNA expression in 101 surgically sectioned GC samples.RESULTS:The TGF-β1 positive expression rate was 47.5%, which was higher in those with lymph node metastasis higher than those without lymph node metastasis (x2 = 4. 4431, P<0.05) and higher in the blood vessel with tumor thrombus than those without (x2=9.8568, P<0.01). The cancer invasion degree in S and Sm, M, Pm and P showed significant difference (x2 = 7.4799, P<0.05. Those with positive PDGF- A had lower 5 and 10 year survival rate than those with negative PDGF-A. This indicated that those with positive PDGF-A had poorer prognosis. There was no correlation in the positive expression between PDGF-A and TGF- β1.CONCLUSIONS: GC with positive PDGF- A and TGF-β1 had higher malignancy, easy metastasis and poorer prognosis. The two might have synergetic effect. PDGF-A may serve as one of the indexes to determine the prognosis of GC.
