青光眼睫状体炎综合征误诊的分析

摘　　要：目的　分析青光眼睫状体炎综合征误诊原因及鉴别诊断要点。方法　选择临床资料及长期随访记录完整的青光眼睫状体炎综合征被误诊的患者 40例 (4 2只眼 )为研究对象 ,进行归类统计、分析及长期随访观察。结果　青光眼睫状体炎综合征被误诊为 :1急性闭角型青光眼 2 0例 (2 0只眼 ) ,占 47.6 2 %;2急性虹膜睫状体炎继发性青光眼 8例 (8只眼 ) ,占 19.0 5 %;3急性虹膜睫状体炎 5例 (5只眼 ) ,占 11.90 %;4中间葡萄膜炎 5例 (7只眼 ) ,占 16 .6 7%;5 Fuchs综合征 2例 (2只眼 ) ,占 4.76 %。长期观察表明 :此综合征每年发作次数不等 ,差别较大 ,因人而异 ;每次发作无明确诱因 ;多次反复发作 ,视功能不受影响。有 1例 (1只眼 ) 10年后合并原发性开角型青光眼发生。结论　青光眼睫状体炎综合征的误诊较为常见 ,必须掌握其诊断与鉴别诊断的要点 ,还需长期观察 ,少数病例可合并原发性开角型青光眼发生。Objective To analyse the reason of misdiagosis and summary the main points of differentiation of glaucomato cyclitic syndrome.Methods 40 misdiagnosed cases (42 eyes) were selected from January,1986 to January,2001 in the hospitals.These cases were kept long-term observations and diagnosed as glaucomato cyclitic syndrome at last.Results By sorting out and analyzing all these cases,we found glaucomato cyclitic syndrome was usually misdiagnosed as:(1)acute angle-closure glaucoma,20 cases (20 eyes);(2)secondary glaucoma caused by acute iridocyclitis,8 cases (8 eyes);(3)acute iridocyclitis,5 cases (5 eyes);(4)intermediate uveitis,5 cases (7 eyes);(5)Fush's syndrome 2 cases (2 eyes).The syndrome is easy to recrudesce,it occurred 3-5 times or so per year and the times were different in indiuiduals.The obvious induced causes were hard to be found.Although it's high recurrence rate,it has no effect on vision.A case (1 eye) was complicated by open-angle glaucoma after 10 years.Conclusions The misdiagosis of glaucomato cyclitic syndrome is very common in clinical.For correct realizing it,we must master the main point of its diagnosis and differentiation and observe the patients for a long time.Open-angle glaucoma can be a complication in a few cases.
