摘　　要：目的 :分析髓母细胞瘤的临床特点、治疗效果及影响预后的因素。方法 :分析 84例经手术和病理证实的髓母细胞瘤病例的临床表现、诊断及综合治疗方法和影响预后因素 ,重点为手术等治疗与预后的关系。结果 :均采用显微手术 ,肿瘤全切 76例 ,近全切 8例 ,术后行脑室 腹腔分流术8例 ,手术死亡率 0 ,6 0例行术后放疗、化疗并随访 ,获得良好疗效。本组 1例术后 2个月死亡 ,术后 2年原位复发 16例 ,转移到其他部位 2 0例。结论 :髓母细胞瘤恶性程度高 ,手术全切肿瘤后辅以足够剂量的放疗为主等综合治疗及定期随访可有效降低复发率和转移率 。Objective Analyzing clinical characteristics of Medulloblastoma?its effects of therapy and the influential factors for prognosis Materials and methods Retrospective analyzing 84 patients with Medulloblastoma demonstrated by pathology and operation,combined with correlative references,concluding their clinical manifestation?diagnosis?and combined treatment?the influential factors prognosis,especially the relationship between surgical excision and prognosis Results All patients were operated by microsurgery Tumors were resected totally in 76 patients,subtotal resection in 8 patients,and postoperative ventriculo celiac shunt in 8 patients Mortality of operation was zero Postoperative radiotherapy or chemotherapy?following up had been done in 60 patients,the effects of therapy were good In this group,one patient died after operation in two months,tumor recurred at original site in 16 patients after operation in two years,and transfered the other sites in 20 patients Conclusions Medulloblastoma is a higher malignant tumor The rate of recurrence and transfer of this tumor can be reduced by combined treatment involving the tumor excised entirely?postoperative radiotherapy and following up periodically,and can prolong survival time
