摘 要:目的:探讨HLA在骨肉瘤细胞上的表达与其生物学意义的关系。方法:采用免疫组化技术对骨肉瘤标本的HLA表达进行检测。结果:骨肉瘤组与正常对照组相比,HLA-Ⅰ类抗原表达下降(P<0.01),HLA-DR抗原表达增强(P<0.01)。典型骨肉瘤组与皮质旁骨肉瘤组相比,HLA-Ⅰ类抗原表达下降(P<0.01),而HLA-DR抗原表达却无统计学差异(P>0.05)。典型骨肉瘤HLA表达与肺转移和生存期之间无统计学差异(P>0.05)。结论:骨肉瘤细胞与癌细胞一样,HLA-Ⅰ类抗原表达降低与其恶性程度相关;HLA-DR抗原有限的表达增强,不能产生有效的免疫应答而杀伤肿瘤细胞。因此,HLA-Ⅰ类抗原降低或缺失是骨肉瘤细胞免疫逃逸的原因之一。Objective:To study the relationship between HLA antigen expressed on osteosarcoma cells and its biological significance. Methods; A three-step immunoperoxidase staining technique was used on paraffi-nembedded sections of 30 osteosarcoma specimens. The stained slides of HLA expression were evaluated semiquantitatively and the relationship between the expressed HLA antigens and related patients' clinical data were statistically analyzed by means of Wilcoxcon test. Results:In the examined 30 osteosarcomas, the reduced or lost expression of HLA class I antigens and the increased expression of HLA-DR antigen are demonstrated with the remarkable statistical significances compared with the 10 normal controls(P<0. 01). The expression of HLA class I antigens on the high grade malignant intramedullary typical osteosarcoma cells is much less than that on the low grade malignant juxtacortical osteosarcoma cells statistically (P<0. 01). While there is no any statistical difference demonstrated in the expression of HLA-DR antigen between the two types of osteosarcoma (P>0. 05). Neither do we find any statistically significant corelation between HLA expression, pulmonary metastases and survival time in typical osteosarcoma (P>0. 05 ).Conclusions : As the same with malignancies derived from the epithelium, osteosarcoma cells expressing reduced HLA class I antigens which is related to their malignant behavior. The limited increased expression of HLA-DR antigen can not elicit the effective immunoreaction for inhibiting or even eradicating osteosarcoma cells. The predominant reason responsible for osteosarcoma cells escaping from immunosurveillance results from the reduced expression of HLA class I antigens.
