摘  要:目的 观察生物活性玻璃 (bioactiveglass,BAG)植入后龈沟液量和弹性蛋白酶水平的变化情况 ,以判断牙周组织的炎症反应。方法 用翻瓣术加BAG移植治疗 13处牙周骨下袋 ,用单纯翻瓣术治疗 7处骨下袋。测定术前及术后 1、2、3、4、6、8、10、12周的龈沟液 (gingivalcrevicularfluid ,GCF)量及GCF中弹性蛋白酶 (elastase ,EA)水平。结果 两组GCF量的变化相似 ,术后 1周GCF量明显升高 ,术后 3周至术前水平 ,6周左右达最低值。BAG组的BCF -EA水平在术后各时间点均较术前明显降低 ;翻瓣组GCF -EA水平在术后 1个月内明显低于术前 ,随后有逐渐回升趋势。术后 12周时 ,BAG组的GCF -EA水平明显低于翻瓣组 (P <0 .0 5 )。结论 BAG可能有助于减轻牙周组织的炎症。Objective To investigate the changes of gingival crevicular fluid (GCF) amount and GCF-EA level after bioactive glass (BAG) grafting and the inflammatory reaction of periodontal tissue.Methods Twenty periodontal intrabony defects were randomly assigned to either a BAG experimental group(open flap debridement with BAG,total 13 defects) or a control OFD (simple open flap debridement,total 7 defects) group.GCF samples were collected with filter paper strips before and 1,2,3,4,6,8,10,12 weeks after surgery.Then the fluid quantity and GCF-EA level were determined by weighing and substrate method,respectively.Results The quantity of gingival crevicular fluid increased significantly at the 1st week after operation then returned to the baseline level at about the 3rd week and reached the lowest point around 6 weeks after surgery.There were no significant differences between treatment groups.GCF-EA levels decreased after surgery in both groups.In BAG group,the GCF-EA levels after surgery were significantly lower than the baseline at all reevaluation points,while in OFD group,only in the first month reevaluations.Compared between the two groups,the GCF-EA level was lower in BAG group but only statistically significant (P< 0.05 ) at the 12th week reevaluation.Conclusion The bioactive glass may be effective in decreasing periodontal inflammation.
