摘 要:目的 为观察围绝经期妇女年骨矿丢失状况、雌激素 (E2 )水平的变化以及二者间的相关性。方法 受试对象为 45~ 50岁尚未绝经的妇女 1 0 0例 ,排除与骨代谢相关的各类疾病。应用双能X线骨密度仪 (美国Lunnar公司 )测定受试人群的正位腰椎及髋部骨密度 ,并应用放射免疫方法测定排卵期血清雌激素含量。均为每年一次 ,连续 3年。结果 第二年各部位骨丢失率腰椎为 1 0 0 % ,股骨颈为 1 1 0 % ,Word s三角区为 1 46 % ,而股骨大转子为 1 2 7% ;第三年比第二年分别降低 1 35 %、1 33 %、1 36 %、1 2 9% ,各部位每年降低 1 0 0 %~ 1 46 % (除第二年股骨颈外 ) ,虽无统计学差异 ,但下降趋势非常明显 ;而血清雌激素水平 ,第二年较第一年大幅降低 31 2 9% ,第三年较第二年又反升2 7 2 8%。结论 围绝经期血清雌激素的水平变化与骨密度逐年下降的趋势无相关性 ,而骨密度下降是增龄、性激素下降以及遗传、运动等其它多种因素共同影响的结果 ,雌激素在这一阶段对BMD未起主导作用。Objective To observe the lose of mineral in bone mass and the changes of estrogen(E 2) level in peri menopausal women and to analyze their correlation Methods One hundred women with the age range of 45~50 years,not yet in the menopausal period and having no illness related with bone metabolism were included in this study.Bone density of lumbar vertebrae and hip joint were assessed with Dual X ray bone density instrument(American Lunnar company);the serum estrogen content in the ovulation periods were detected with RIA;the examinations were carried out annually for 3 years.Results In the 2nd year,the rate of bone lose in lumbar vertebrae was 1 00%,the neck of femur 1 10%,trochanteric area 1 27%;In comparison with the data of the second year,the rate of bone lose in the 3rd year in the differnet area were respectively 1 35%,1 33% and 1 29%.The annual rate of reduction in the different location was 1 00%~1 46% with the exception of the femoral neck in the second year.A descending tendency is obvious though the figures were of no statistical significance.The serum estrogen level however,reduced for a large scale of 31 29% in the 2nd year than that in the 1st year and increased to 27 28% in the 3rd year than data of the 2nd year.Conclusion There were no correlation in bone lose and serum estrogen level in women of the peri menopausal period.The decrease of bone density might be resulted from multiple factors of aging,decrease of estrogen level,hereditary,physical exercise etc.;estrogen did not seem to play the dominant role for the changes of BMD.
