Profile机用根管器械治疗磨牙牙周牙髓联合病变的临床效果

摘　　要：【目的】探讨牙周牙髓联合病变的牙髓活力状况与牙髓治疗状况对临床治疗效果的影响。【方法】选择伴有牙髓病变的牙周炎患牙 12 3个磨牙 ,术前测验牙髓活力 ;以MailleferProfile机用根管器械采用逐步深入法预备根管共 6 0个牙 ;以ManiReamers手用根管器械采用逐步后退法预备根管共 6 3个牙 ;通过术前、术后 (3个月、6个月和 1年 )的X线牙片和临床指标 (探诊出血和深度、附着丧失和牙动度等 )进行对比分析。【结果】术前牙髓活力诊断正常的 10例术后符合率为 30 0 %、敏感的 70例为 84 3%、迟钝的 4 3例为 86 0 % ;MailleferProfile镍钛合金机用根管锉根管偏移率和根管治疗临床效果优于ManiReamers手用根管器械组 ;治疗后 1年的探诊出血指数、6个月及 1年的探诊深度 ,3个月至 1年的附着丧失和牙动度 ,均有明显差异 ,P <0 0 5。【结论】牙髓治疗的时机和效果对牙周牙髓联合病变的临床疗效影响明显 ,MailleferProfile镍钛合金机用根管锉的成形作用好 。To analyse the effects of pulp conditions and root canal therapy condition on therapeutic result of combined periodontal endodontic lesions 123 molars (92 patients)with periodontitis associated with endodontic lesions were selected; the pulp vitality testing was determined before treatment 60 molars' root canals were prepared with Maillefer Profile by step down technique and 63 molars' root canal were prepared with Mani Reamers by step back technique X ray photograph, periodontitis indexes (bleeding upon probing, probing pocket depth, clinical attachment loss and tooth mobile) were compared before and after (3,6 months and l year) root canal and periodontal therapy The accordance rates of the pulp vitality testing in 10 molars diagnosed as normal, 70 molars diagnosed as supersensitive and 43 molars diagnosed as logy were 30 0%, 84 3% and 86 0%, respectively Significant differences of periodontitis indexes were found between Maillefer Profile and Mani Reamers in terms of bleeding upon probing after 1 year, probing pocket depth after 6 months and l year and clinical attachment loss and tooth mobile after 3,6 months and l year (P<0 05) Moreover, Maillefer Profile was superior to Mani Reamers in terms of the operating time, preparation shaping ability (deviation rate) and clinical efficacy [Conclusions] The pulpal conditions are relative to periodontal endodontic lesions The clinical efficacy of periodontal and endodontic management with Maillefer Profile is better than that with Mani Reamers
