摘　　要：目的　研究纳洛酮治疗肺性脑病的效应 ,并探讨其作用机理。方法　选择 48例 COPD并肺脑患者为治疗组。随机另选46例同期住院 COPD并肺脑患者为对照组。在综合治疗措施相同 ,治疗组加用盐酸纳洛酮。结果　治疗组应用纳洛酮 2 4h后 ,与对照组比较。 SBP[(12 0 .3± 8.78) mm Hg比 (95 .2 5± 7.8mm Hg],SPO2 [(96 .6 8± 2 .39) %比 (92± 3.18) % ],Pa O2 [(89.6 1± 10 .2 5 )mm Hg比 (75 .6 8± 12 .78) mm Hg]显著升高 ,P<0 .0 1。 HP[(81.18± 7.98)次 /分比 (91.0 2± 8.6 2 )次 /分 ]R[(2 1.12± 3.0 8)次 /分比(2 5 .32± 8.6 6 )次 /分 ],显著降低 ,P<0 .0 1.结论　纳洛酮治疗肺性脑病 ,能使昏迷和呼吸抑制的病人快速逆转意识障碍 。Objectve To investigate the curative effect of naloxone on pulmonary encephlopathy and its possible mechanism.Methods 48 Patients with chronic obstructive pulmonary disease(COPD) and pulmonary encephalopathy(PE) were selected as treatment group(30 male,18 female,average age 63.5±5.6),Who had naloxne.46 patients with COPD and PE were selected as contrl,(30 male,16 female,averay age 62.5±4.8).Without naloxne.The two groups were compared on systolic pressure(SBP),heart rate (HR),respiration(R).oxygen saturation(SO 2),arterial blood gas (ABG) partial pressure of oxygen(PaO 2),Partial pressure of carbon dioxide (PaCO 2).Results 24 hours after using nalozone,data the two groups were analyzed and compared.Compared with the controls.The treatment group had significantly higher.SBP(12.03±8.78)mmHg vs 95.25±7.8mmHg),SPO 2(96.68±2.39% vs 92±3.18%,PaO 2(89.61±10.25)mmHg vs (75.68±12.78)mmHg(P<0.01);and lowes HR(81.16±7.98 vs 91.02±8.62)and R(21.12±3.08 vs 25.32±8.66) (P<0.01).Conclusion Naloxone therepy on pulmonany encephalopathy can make coma and repiration inhibited patients rapidly reverse their dysconsciousness and ,relieve their respiration inhibition.The possible mechanism that naloxone produces includes cerebral perfusive pressure when brain is short of blood,lohich reverses the impairment of endogenous morphine and improve encephalic metablism.
