摘　　要：目的　探讨早期胃癌的淋巴结转移规律及其对预后的影响。方法　对 16 1例早期胃癌病人术后进行长期随访 ,对 2 4例伴有淋巴结转移的早期胃癌与 137例无淋巴结转移的早期胃癌的临床病理特征及 3、5年生存率进行比较。结果　早期胃癌的淋巴结转移与肿瘤大小、浸润深度及淋巴、静脉侵犯有关 ,伴有淋巴结转移的早期胃癌 3、5年生存率分别为 82 8%和 80 5 % ,明显低于无淋巴结转移者 ,后者分别为 96 1%和 92 4%。Objective To explore the general pattern of lymph node metastasis and its effect on prognosis in early gastric cancer.Methods After surgery 161 patients with early gastric cancer were followed up for a long time.The clinicopathologic features and the 3- and 5-year survival rates of 24 patients with node-positive early gastric cancer were compared with those of 137 patients with node-negative early gastric cancer.Results The tumor size,depth of invasion,and lymphatic and venous involvement were related to lymph node metastasis in early gastric cancer.The 3- and 5-year survival rates of patients with node-positive early gastric cancer were 82^8% and 80^5% respectively,which were significantly lower than those of patients with node-negative early gastric cancer,96^1% and 92^4% respectively.Conclusion Precise evaluation of lymph node metastasis pre- or intraoperatively is essential for therapeutic options and prognosis improvement in early gastric cancer.
